植入式静脉输液港与PICC在乳腺癌术后辅助化疗中的应用效果比较

Comparison of application effects of implanted venous port and PICC in postoperative adjuvant chemotherapy for breast cancer

目的比较植入式静脉输液港与PICC在乳腺癌术后辅助化疗中的应用效果。方法回顾性分析2017年6月至2020年6月于济宁医学院附属医院乳腺外科进行乳腺癌术后化疗的544例患者的临床资料,根据术后辅助化疗的输液方式将其分为输液港组(248例)和PICC组(296例)。化疗结束后,比较两组患者的置管成功情况、并发症发生情况、满意度及生活质量。结果两组的置管成功情况比较,差异无统计学意义(P>0.05)。输液港组的并发症总发生率低于PICC组,差异具有统计学意义(P<0.05)。输液港组的满意度高于PICC组,差异具有统计学意义(P<0.05)。输液港组的SF-36评分高于PICC组,差异具有统计学意义(P<0.05)。结论相较于PICC,植入式静脉输液港应用于乳腺癌术后辅助化疗中可提高一次置管成功率,降低置管相关并发症发生率,保证化疗顺利进行,提高患者的治疗满意度及生活质量。

Objective To compare the application effects of implanted venous port and PICC in postoperative adjuvant chemotherapy for breast cancer.Methods The clinical data of 544 patients who received postoperative chemotherapy for breast cancer in the breast surgery department of affiliated hospital of Jining medical university from June 2017 to June 2020 were retrospectively analyzed.According to the infusion method of postoperative adjuvant chemotherapy,the patients were divided into infusion port group(248 cases)and PICC group(296 cases).After chemotherapy,the success of catheterization,occurrence of complications,satisfaction and quality of life were compared between the two groups.Results There was no significant difference in the success of catheterization between the two groups(P>0.05).The total incidence of complications in the infusion port group was lower than that in the PICC group,and the difference was statistically significant(P<0.05).The satisfaction of the infusion port group was higher than that of the PICC group,and the difference was statistically significant(P<0.05).The SF-36 score of the infusion port group was higher than that of the PICC group,and the difference was statistically significant(P<0.05).Conclusion Compared with PICC,the implanted venous port applied in postoperative adjuvant chemotherapy for breast cancer can improve the success rate of one-time catheterization,reduce the incidence of catheterization related complications,ensure the smooth progress of chemotherapy,and improve patients'satisfaction with treatment and quality of life.
