血清LDH检测在恙虫病患者早期诊断中的应用价值

Application value of serum LDH detection in early diagnosis of patients with tsutsugamushi disease

目的探究血清乳酸脱氢酶(LDH)检测在恙虫病患者早期诊断中的应用价值。方法选择2017年5月至2020年5月我院56例羌虫病患者作为研究对象,将其设为观察组,再选择同期我院50名健康体检者作为对照组。检测两组血小板(PLT)计数、血清LDH、丙氨酸氨基转移酶(ALT)以及天冬氨酸氨基转移酶(AST)水平并比较;分析观察组患者血清LDH水平与PLT计数及血清ALT、AST水平的相关性;比较观察组患者治疗前、后的PLT计数及血清LDH、ALT、AST水平。结果观察组患者的临床症状主要集中在发热、头痛以及腹痛方面,占比分别为92.86%、71.43%、67.86%。观察组的PLT计数显著少于对照组,血清LDH、ALT以及AST水平显著高于对照组(P<0.05)。Pearson相关性分析结果显示,观察组患者血清LDH水平与PLT计数呈负相关(r=-0.943,P<0.05),与ALT、AST呈正相关(r=0.964、0.941,P<0.05)。治疗后,观察组的PLT计数显著多于治疗前,血清LDH、ALT以及AST水平显著低于治疗前(P<0.05)。结论血清LDH在恙虫病患者中呈现出高表达,且与患者PLT、ALT及AST指标密切相关,能够作为恙虫病早期诊断的敏感性指标,可予以推广。

Objective To explore the application value of serum lactate dehydrogenase(LDH)detection in early diagnosis of patients with tsutsugamushi disease.Methods From May 2017 to May 2020,56 patients with tsutsugamushi disease in our hospital were selected as the study objects and seted as observation group,and 50 healthy people in the same period were selected as control group.Platelet(PLT)count,serum LDH,alanine aminotransferase(ALT)and aspartate aminotransferase(AST)levels were measured and compared between the two groups;the correlation between serum LDH level and PLT count,the levels of serum ALT,AST in the observation group were analyzed;the PLT count and the levels of serum LDH,ALT and AST in the observation group were compared before and after treatment.Results The clinical symptoms in the observation group mainly focused on fever,headache and abdominal pain,accounting for 92.86%,71.43%and 67.86%respectively.The PLT count in the observation group was significantly less than that in the control group,and the levels of serum LDH,ALT and AST were significantly higher than those in the control group(P<0.05).Pearson correlation analysis result showed that the serum LDH level in the observation group was negatively correlated with PLT count(r=-0.943,P<0.05),and positively correlated with ALT and AST(r=0.964,0.941,P<0.05).After treatment,the PLT count in the observation group was significantly more than that before treatment,and the levels of serum LDH,ALT and AST were significantly lower than those before treatment(P<0.05).Conclusion Serum LDH is highly expressed in patients with tsutsugamushi disease,and is closely related to the PLT,ALT and AST indexes of patients.It can be used as a sensitive index for early diagnosis of tsutsugamushi disease and can be promoted.
