彩色多普勒超声引导下凝血酶注射法治疗医源性股动脉假性动脉瘤的临床价值

Clinical value of thrombin injection guided by color Doppler ultrasound in the treatment of iatrogenic femoral artery pseudoaneurysm

目的探讨彩色多普勒超声引导下凝血酶注射法治疗医源性股动脉假性动脉瘤的临床价值。方法选取2017年5月至2019年9月于陕西中医药大学附属医院确诊并行凝血酶注射治疗的医源性股动脉假性动脉瘤患者18例。回顾性分析治疗效果及不良反应发生情况。结果12例患者一次栓塞成功;4例患者一次注射凝血酶并按压后,瘤体内仍可见血流信号,行二次注射,治疗成功;1例患者注射后,瘤体内仍可见血流信号,患者出现下肢发热、麻木,停止注射;1例复杂性股动脉假性动脉瘤患者,分两次行超声下引导凝血酶注射术,两个瘤腔依次闭塞。结论彩色多普勒超声引导下凝血酶注射治疗医源性股动脉假性动脉瘤操作简便、创伤小、成功率高,具有较高的临床应用价值。

Objective To explore the clinical value of thrombin injection guided by color Doppler ultrasound in the treatment of iatrogenic femoral artery pseudoaneurysm.Methods Eighteen patients with iatrogenic femoral artery pseudoaneurysm diagnosed and treated with thrombin injection in the affiliated hospital of Shaanxi university of Chinese medicine from May 2017 to September 2019 were selected.The therapeutic effect and adverse reactions were analyzed retrospectively.Results Twelve patients were successfully embolized at one time;blood flow signals still existed in 4 patients after one injection of thrombin and compression,and the patients were given second injection and successfully cured;blood flow signals still existed in a patient after thrombin injection,the patient developed fever and numbness of lower limbs,and the injection were stopped;a patient with complex femoral artery pseudoaneurysm was given ultrasound-guided thrombin injection twice,and the two tumor cavities were occluded in turn.Conclusion The thrombin injection guided by color Doppler ultrasound in the treatment of iatrogenic femoral artery pseudoaneurysms has the advantages of simple operation,less trauma and high success rate,and high clinical application value.
