经尿道前列腺等离子双极电切术治疗良性前列腺增生对降低并发症发生率的意义

Significance of Transurethral Plasma Bipolar Resection of Prostate for Benign Prostatic Hyperplasia in Reducing the Incidence of Complications

目的分析良性前列腺增生经尿道前列腺双极等离子电切术对降低并发症发生率的意义。方法选取该院2018年10月—2019年10月收治的84例良性前列腺增生患者,根据手术方式不同分为对照组(经尿道前列腺电切术,n=42)和观察组(经尿道前列腺等离子双极电切术,n=42)。比较两组手术相关指标、前列腺症状评分以及术后并发症发生率。结果观察组手术总用时(51.20±14.56)min、置管时间(4.42±1.88)d及膀胱冲洗时间(3.12±1.22)d均较对照组[(68.23±15.52)min、(6.30±2.87)d、(4.72±2.40)d]短,观察组失血量(80.53±20.35)L较对照组(114.32±21.23)L少,差异有统计学意义(t=5.186、3.551、3.851、7.446,P<0.05)。术后3个月,观察组IPSS(7.11±0.86)分,较对照组(12.48±1.45)分低,差异有统计学意义(t=20.643,P<0.05)。观察组术后并发症总发生率[7.14%(3/42)]较对照组[23.81%(10/42)]低,差异有统计学意义(χ^(2)=4.459,P=0.035)。结论良性前列腺增生经尿道前列腺双极等离子电切术治疗可缩短术后恢复时间,改善前列腺症状,降低术后并发症发生率,值得临床推广应用。

Objective To analyze the significance of transurethral bipolar plasma resection of the prostate for benign prostatic hyperplasia in reducing the incidence of complications.Methods The clinical data of 84 patients with benign prostatic hyperplasia admitted in our hospital from October 2018 to October 2019 were selected and divided into a control group(transurethral resection of the prostate,n=42)and an observation group(transurethral plasma bipolar resection of the prostate,n=42).The two groups were compared with surgery-related indicators,prostate symptom scores,and postoperative complications.Results The total operation time of the observation group was(51.20±14.56)min,the catheterization time(4.42±1.88)d,and the bladder irrigation time(3.12±1.22)d were all compared with the control group[(68.23±15.52)min,(6.30±2.87)d,(4.72±2.40)d]being short,the blood loss of the observation group was(80.53±20.35)L less than the control group(114.32±21.23)L,the difference was statistically significant(t=5.186,3.551,3.851,7.446,P<0.05).Three months after the operation,the IPSS(7.11±0.86)points of the observation group was lower than that of the control group(12.48±1.45)points,the difference was statistically significant(t=20.643,P<0.05).The total incidence of postoperative complications in the observation group was 7.14%(3/42)lower than 23.81%(10/42)in the control group,and the difference was statistically significant(χ^(2)=4.459,P=0.035).Conclusion The treatment of benign prostatic hyperplasia with transurethral resection of the prostate can shorten the postoperative recovery time,improve the prostate symptoms,and reduce the incidence of postoperative complications.It is worthy of clinical application.
