骨外固定架联合负压封闭引流治疗创伤后骨髓炎的疗效评价

Evaluation of the Curative Effect of External Bone Fixator Combined with Negative Pressure Sealing Drainage on Post-traumatic Osteomyelitis

目的分析骨外固定架联合负压封闭引流治疗创伤后骨髓炎的疗效。方法纳入该院符合标准的100例创伤后骨髓炎患者进行观察,患者在2017年1月—2020年4月期间入组,按照时间顺序分为观察组和对照组,每组50例。对照组使用常规置管引流,观察组使用骨外固定架联合负压封闭引流治疗,对比两组的恢复效果、治疗效果和并发症。结果观察组局部发热消失时间是(3.05±1.15)d,红肿消失时间是(2.88±1.15)d,住院时间(32.65±6.27)d,与对照组比较,差异有统计学意义(t=6.480、5.223、7.943,P<0.05);观察组关节功能恢复优良率是96%,高于对照组的80%,差异有统计学意义(χ^(2)=6.061,P<0.05);观察组并发症发生率数据是2%,对照组是14%,差异有统计学意义(χ^(2)=4.891,P<0.05)。结论骨外固定架联合负压封闭引流治疗创伤后骨髓炎可以促进发热、红肿等症状的消退,并且治疗效果好,恢复更快,并发症少,建议推广。

Objective To analyze the curative effect of external bone fixation combined with negative pressure sealing drainage in the treatment of post-traumatic osteomyelitis.Methods We included 100 patients with post-traumatic osteomyelitis who met the criteria in the hospital for observation.The patients were enrolled in the group from January 2017 to April 2020,and they were grouped into observation group and contorl group in chronological order.50 cases in each group.The control group used conventional catheter drainage,and the observation group used external bone fixator combined with negative pressure sealing drainage treatment,the recovery effect,treatment effect and complications of the two groups were compared.Results The disappearance time of local fever in the observation group was(3.05±1.15)d,the disappearance time of redness and swelling was(2.88±1.15)d,and the hospital stay was(32.65±6.27)d,compared with the control group,the difference was statistically significant(t=6.480,5.223,7.943,P<0.05).The excellent rate of joint function recovery in the observation group was 96%,which was higher than 80%in the control group.The difference between the groups was statistically significant(χ^(2)=6.061,P<0.05);the complication rate of the observation group was 2%,and the control group was 14%.The difference between the groups was statistically significant(χ^(2)=4.891,P<0.05).Conclusion The treatment of post-traumatic osteomyelitis with external fixation frame combined with negative pressure can promote the resolution of fever,redness and swelling,and the treatment effect is good,the recovery is faster,and the complications are less.It is recommended to promote it.
