抗瓜氨酸化α烯醇化酶多肽1抗体在初治类风湿关节炎患者中的表达及其临床价值研究

The Expression and Clinical Value of Anti-citrullinatedα-enolase Polypeptide 1 Antibody in Newly Treated Patients with Rheumatoid Arthritis

目的探究抗瓜氨酸化α烯醇化酶多肽1(CEP-1)抗体在初治类风湿关节炎(RA)患者中的表达及其临床价值。方法回顾性分析2019年4月—2020年5月于该院行相关检查的50例初治RA患者、30名健康体检者临床资料,将RA患者纳入RA组,将健康体检者纳入健康对照组,比较两组抗CEP-1抗体表达情况;根据抗CEP-1抗体阳性与否将50例初治RA患者分为阳性组(n=33,抗CEP-1抗体阳性表达者)和阴性组(n=17,抗CEP-1抗体阴性表达者),比较两组28个关节疾病活动度评分(DAS28)、改良Sharp评分,采用Spearman相关性分析法分析初治RA患者抗CEP-1抗体表达与DAS28、改良Sharp评分的相关性,采用Logistic多元回归分析法分析初治RA患者抗CEP-1抗体表达独立风险因素。结果RA组抗CEP-1抗体阳性率为66%,显著高于健康对照组的0%,差异有统计学意义(P<0.05)。初治RA患者中,阳性组DAS28、改良Sharp评分显著高于阴性组,差异有统计学意义(t=6.548、6.008,P<0.05);Spearman相关分析结果表明,初治RA患者抗CEP-1抗体表达与DAS28、改良Sharp评分呈正相关(P<0.05);经Logistic多元回归分析法分析得出,DAS28(6.05±1.01)points、改良Sharp评分(76.21±14.54)points均可受到初治RA患者抗CEP-1抗体表达的独立影响(P<0.05)。结论抗CEP-1抗体在RA患者中多表达出阳性表现,其与初治RA患者DAS28、改良Sharp评分具有显著相关性,可用于评估此类患者临床关节活动度及骨破坏程度。

Objective To explore the expression and clinical value of anti-citrullinated alpha enolase polypeptide 1(CEP-1)antibody in newly treated rheumatoid arthritis(RA)patients.Methods A retrospective analysis of the clinical data of 50 newly-treated RA patients and 30 healthy subjects who underwent relevant examinations in the hospital from April 2019 to May 2020.RA patients were included in the RA group,and healthy subjects were included in the healthy control group.The two groups were compared of CEP-1 antibody expression;according to whether the anti-CEP-1 antibody was positive or not,50 newly treated RA patients were divided into positive group(n=33,positive expression of anti-CEP-1 antibody)and negative group(n=17,anti-CEP-1 antibody negative expression),the 28 joint disease activity scores(DAS28)and modified Sharp scores were compared between the two groups,and the Spearman correlation analysis method was used to analyze the expression of anti-CEP-1 antibodies with DAS28 and modified Sharp scores in newly treated RA patients;the Logistic multiple regression analysis method was used to analyze the independent risk factors of anti-CEP-1 antibody expression in newly treated RA patients.Results The positive rate of anti-CEP-1 antibody in the RA group was 66%,which was significantly higher than 0%in the healthy control group,the difference was statistically significant(P<0.05).In newly-treated RA patients,the DAS28(6.05±1.01)points and modified Sharp scores(76.21±14.54)points of the positive group were significantly higher than those of the negative group,the difference was statistically significant(t=6.548,6.008,P<0.05);the results of Spearman correlation analysis showed that the expression of anti-CEP-1 antibody in newly-treated RA patients was correlated with DAS28,modified Sharp score was positively correlated(P<0.05);Logistic multiple regression analysis showed that DAS28 and modified Sharp score could be independently affected by the expression of anti-CEP-1 antibody in newly treated RA patients(P<0.05).Conclusion Anti-CEP-1 antibody is more positively expressed in RA patients,and it is significantly correlated with DAS28 and modified Sharp score in newly treated RA patients,and can be used to assess the degree of clinical joint mobility and bone destruction in such patients.
