The relationship between the expressions of ERCC1,RRM1,and BRCA1 in non-small cell lung cancer tissues and the prognosis

摘要 目的探讨肺癌组织错配切除修复蛋白1(ERCC1)、核苷酸还原酶M1(RRM1)和乳腺癌易感基因1(BRCA1)表达与预后的关系。方法2014年1月至2016年1月河南省周口市第五人民医院行肺癌根治术的72例非小细胞肺癌(NSCLC)患者,按手术切除肿瘤组织和癌旁组织分组,采用免疫组化法检测ERCC1、RRM1、BRCA1水平,并采用实时荧光定量聚合酶链反应(PCR)检测BRCA1 mRNA表达;比较不同ERCC1、RRM1、BRCA1水平和不同BRCA1 mRNA表达的NSCLC患者临床病理资料、预后情况。结果NSCLC患者癌组织ERCC1、RRM1、BRCA1蛋白表达阳性率及BRCA1 mRNA相对表达量均高于癌旁组织(P<0.05);淋巴结转移率比较ERCC1阳性>ERCC1阴性、RRM1阳性>RRM1阴性,差异有统计学意义(P<0.05);BRCA1阳性与BRCA1阴性患者淋巴结转移率比较,差异无统计学意义(P>0.05);ERCC1、RRM1阳性者与阴性者、BRCA1 mRNA高表达者与低表达者的3年总生存率、3年无病生存率、中位生存期比较,差异均有统计学意义(P<0.05)。结论ERCC1、RRM1和BRCA1均参与肺癌发生、进展,三者表达情况在一定程度上可预测预后状况,有利于临床治疗方案制定。 Objective To explore the relationship between the expressions of excision repair cross complement 1(ERCC1),ribonucleotide reductase M1(RRM1)and breast cancer susceptibility gene 1(BRCA1)in lung cancer tissues and the prognosis.Methods Clinicopathological data of 72 patients with non-small cell lung cancer(NSCLC)who underwent radical resection of lung cancer from January 2014 to January 2016 in our hospital were retrospectively analyzed.The patients were grouped according to surgical excision of cancer tissues and adjacent tissues.Immunohistochemistry was used to detect the levels of ERCC1,RRM1,and BRCA1.Real-time fluorescence quantitative polymerase chain reaction(PCR)was used to detect the BRCA1 mRNA expression.The clinicopathological data and the prognosis were compared among NSCLC patients with different levels of ERCC1,RRM1 and BRCA1 and different BRCA1 mRNA expressions.Results There were statistically significant differences in the positive expression rates of ERCC1,RRM1 and BRCA1 proteins and relative expression levels of BRCA1 mRNA in cancer tissues and adjacent tissues of the NSCLC patients(P<0.05).There were statistically significant differences in the positive rate and negative rate of ERCC1,the positive rate and negative rate of RRM1 and the high expression rate and low expression rate of BRCA1 mRNA in lymph node metastasis of NSCLC patients(P<0.05).There were statistically significant differences in the 3-year overall survival rate,3-year disease-free survival rate,and median survival rate between patients with positive and negative ERCC1 and RRM1 and between patients with high and low BRCA1 mRNA expression(P<0.05).Conclusion ERCC1,RRM1,and BRCA1 are all involved in the occurrence and progression of NSCLC.Their expressions can predict the prognosis status to a certain extent which is conducive to the formulation of clinical treatment regimen.
