骨科患者围术期白蛋白、C反应蛋白变化与术后感染性并发症的关系分析

The changes of albumin and C-reactive protein in orthopedic patients during perioperative period and analysis of their relationship with postoperative infectious complications

目的探讨骨科手术患者围术期白蛋白(Alb)、C反应蛋白(CRP)水平变化,分析其与术后感染并发症的相关性。方法选取本院收治的骨科手术患者555例,依据是否伴有术后并发症分为并发症组和无并发症组,回顾性分析两组临床资料。结果并发症组年龄、ASA分级、基础疾病者占比、CRP高于无并发症组,围术期Alb、Alb/CRP低于无并发症组(P<0.05);Alb/CRP是骨科患者术后并发症的发生独立危险因素(P<0.05);Alb/CRP对骨科术后并发症预测的AUC最大(P=0.042),二者比值对骨科患者术后并发症的预测效果优于单一检测(P<0.05)。结论Alb、CRP比值是骨科患者术后并发症发生的独立危险因素,可作为预测不良临床的有效指标。

Objective To investigate the changes of albumin(Alb)and C-reactive protein(CRP)in patients during orthopedic surgery,and analyze their correlation with postoperative infectious complications.Methods A total of 555 patients who underwent orthopedic surgery in our hospital were selected.The patients were divided into complication group and non-complication group according to presence or absence of complications after surgery.Clinical data of the two groups were retrospectively analyzed.Results Age,classification of American Society of Anesthesiologists(ASA),proportion of patients with underlying diseases and CRP in the complication group were higher than those in the non-complication group(P<0.05).However,perioperative Alb and Alb/CRP ratio in the complication group were significantly lower than those in the non-complication group(P<0.05).The Alb/CRP ratio was an independent risk factor for postoperative complications in orthopedic patients(P<0.05).The AUC of ROC curve analysis for Alb/CRP ratio was the greatest for predicting postoperative complications(P=0.042),and the predictive effect was better than single detection(P<0.05).Conclusion The Alb/CRP ratio is an independent risk factor for postoperative complications in orthopedic patients.It can be applied as an effective index for predicting poor clinical outcomes.
