济南市2019年基层医疗机构消毒质量监测分析

Analysis of disinfection quality monitoring of primary medical institutions in Jinan in 2019

目的了解济南市基层医疗机构消毒质量现状,找出消毒卫生工作中存在薄弱环节,提出改进措施,为预防控制医院感染提供依据。方法按照《医院消毒卫生标准》等相关标准和规范进行抽样监测,并对监测结果进行统计分析。结果2019年共监测基层医疗机构107所,采集样品2061份,合格1728份,合格率为83.84%。县(区)级医院、社区卫生服务中心、民营医院、乡镇卫生院、个体诊所合格率分别为84.99%、84.64%、84.33%、81.96%、81.64%;不同监测项目合格率分别为:空气80.14%、物体表面84.85%、卫生手84.07%、外科手91.55%、使用中消毒剂100.00%、紫外线灯90.10%、医疗用水76.34%、医院污水94.12%、压力蒸汽灭菌器100.00%、内镜冲洗水80.95%,差异均有统计学意义(P<0.01)。结论2019年济南市基层医疗机构消毒合格水平有待进一步提高,尤以医疗用水、室内空气、内镜冲洗水及口腔用水为甚,建议相关职能部门加强基层医疗机构消毒与感染知识培训,提高监督检测频率,基层医疗机构应健全完善消毒与感染控制监管工作体系,落实消毒管理制度,做好日常消毒工作,提高消毒质量有效预防院内感染。

Objective To understand the current situation of disinfection quality in primary medical institutions in Jinan,find out the weak links in disinfection and sanitation work,and put forward improvement measures,so as to provide basis for the prevention and control of hospital infection.Methods The sampling monitoring was carried out according to the relevant standards and norms such as Hospital Disinfection Health Standard,and the monitoring results were statistically analyzed.Results In 2019,107 primary medical institutions were monitored,2061 samples were collected,1728 were qualified,and the qualified rate was 83.84%.The qualified rates of County(District)hospitals,community health service centers,private hospitals,township health centers and individual clinics were 84.99%,84.64%,84.33%,81.96%and 81.64%respectively.The qualified rates of different monitoring items were as follows:air80.14%,object surface 84.85%,sanitary hand 84.07%,surgical hand 91.55%,disinfectant in use 100.00%,ultraviolet lamp 90.10%,medical water 76.34%,hospital sewage 94.12%,pressure steam sterilizer 100.00%,endoscopic flushing water 80.95%,the differences were statistically significant(P<0.01).Conclusion In2019,the disinfection qualification level of primary medical institutions in Jinan City needed to be further improved,especially medical water,indoor air,endoscopic flushing water and oral water.It was suggested that relevant functional departments should strengthen the training of disinfection and infection knowledge in primary medical institutions,improve the frequency of supervision and detection.Primary medical institutions should improve the supervision system of disinfection and infection control,and implement disinfection management system,do a good job in daily disinfection work,improve the quality of disinfection,effectively prevent nosocomial infection.
