2015-2019年四川省50岁及以上HIV/AIDS病例晚发现情况及影响因素分析

Characteristics and Influencing Factors of Late-diagnosis HIV/AIDS Cases Aged≥50 Years During 2015 to 2019 in Sichuan Province

目的了解四川省50岁及以上HIV/AIDS病例晚发现情况及相关影响因素。方法根据中国艾滋病综合防治信息系统中四川省2015-2019年新报告HIV/AIDS病例信息,分析不同特征HIV/AIDS病人晚发现状况,并运用χ^(2)检验、二分类Logistic回归分析模型分析50岁及以上HIV/AIDS病例晚发现的相关影响因素。结果2015-2019年四川省新报告的50岁及以上HIV/AIDS病例报告数和占比依次为5951例(34.31%)、8133例(39.22%)、10681例(43.91%)、16201例(46.70%)和22047例(59.64%);历年的晚发现病例数和占比依次为2673例(44.92%)、3320例(40.82%)、4338例(40.61%)、6651例(41.05%)和8641例(39.19%);性别、婚姻状况、年龄、样本来源是HIV/AIDS病例晚发现的影响因素,男性、离异或丧偶、高年龄、重点人群和羁押人群,50岁及以上HIV/AIDS病例晚发现风险更高(均有P<0.001)。结论50岁及以上人群已成为四川省HIV感染重点人群;晚发现率在2015年下降后保持平稳,应针对50岁及以上晚发现高风险人群制定针对性的检测策略,尽快发现感染者。

Objective To analyze the characteristics and influencing factors of late-diagnosed HIV/AIDS cases aged≥50 years during 2015-2019 in Sichuan province.Methods Newly reported HIV/AIDS cases aged≥50 years during 2015-2019 in Sichuan province were selected from databases from the National Information System for Comprehensive HIV/AIDS Prevention and Control to analyze the epidemiological characteristics of late-diagnosed cases.Chi-square tests and binary Logistic regression models were performed to explore the factors influencing the cases with late diagnosis.Results The number of newly reported HIV/AIDS cases of≥50 years in Sichuan Province from 2015 to 2019 were 5951(34.31%),8133(39.22%),10681(43.91%),16201(46.70%)and 22047(59.64%),respectively.The number of late diagnosis HIV/AIDS cases from 2015 to 2019 were 2673(44.92%),3320(40.82%),4338(40.61%),6651(41.05%)and 8641(39.19%),respectively.Gender,marital status,age and sample source were influencing factors for the late diagnosis of HIV/AIDS cases.Males,divorced or widowed,aged≥50 years,key population and custodial population were associated with higher risk of late diagnosis of HIV/AIDS(all P<0.001).Conclusion People aged≥50 years have become the focus group of HIV infection in Sichuan Province.The rate of late-diagnosed cases remained stable after the decline in 2015.Targeted detection strategies should be developed for people aged≥50 years who were at high risk of late diagnosis;thus,infected people can be detected as soon as possible.
