周围性面瘫患者健康相关生命质量的研究及影响因素的调查

The Status and Influence Factors of Health-Related Quality of Life in Patients with Peripheral Facial Paralysis

目的调查周围性面瘫患者健康相关生命质量(HRQOL)的现状及其影响因素。方法对我院针灸科的131例周围性面瘫患者进行问卷调查。采用面部评价量表(FaCE)及面部残疾指数(FDI)(包括躯体功能FDIP和社会生活功能FDIS)的总分测量HRQOL总分,对患者的一般资料进行统计,同时使用Kruskal-Wallis H检验及多元逐步线性回归统计方法对HRQOL的影响因素进行分析。结果影响HRQOL得分的因素有:职业、职业状态、病变部位、HADS-A、年龄、开放性人格、宜人性人格、尽责性人格(P<0.05)。FaCE、FDIP和FDIS得分与职业、病变部位、职业状态、HADS-A、年龄呈负相关(P<0.05);FaCE得分和FDIS与宜人性人格、开放性人格、尽责性人格呈正相关(P<0.05)。结论周围性面瘫患者的HRQOL得分偏低,应重点关注对患者社会生活和人格因素方面的影响,在对患者做好疾病护理的同时,应加强患者的心理的疏导和社会的支持。

Objective To investigate the status and influence factors of health-related quality of life(HRQOL)of patients with peripheral facial paralysis.Methods questionnaire surveys were conducted on 131 patients with peripheral facial paralysis in the department of acupuncture,affiliated hospital of Chengdu university of traditional Chinese medicine.The total score of HRQOL was measured by the total score of the facial clinimetric evaluation(FaCE)scale and facial disability index(FDI)(including FDI physical score and FDI social score).The general data of the patients were collected.Multiple stepwise linear regression was used to analyze the influence factors of HRQOL.Results The factors affecting HRQOL scores were occupation,personal income,lesion location,age,marital status,occupational status,and course of disease(P<0.05).Among them,occupation was negatively correlated with FaCE total score,laterality of facial palsy was negatively correlated with FDIP score,and occupational status,HADS-A,and age were negatively correlated FDIS scores(P<0.05).Open personality was positively associated with FaCE total score,and pleasant personality,open personality and conscientious personality were positively associated with FDI social score(P<0.05).Conclusion The HRQOL score of patients with peripheral facial paralysis is low.Our results showed that more attention should be paid to the patients'social life and personality factors.While taking good care of the patients with peripheral facial paralysis,the psychological counseling and social support of the patients should be strengthened.
