闭合及切开复位交锁髓内钉内固定治疗不同年龄段胫骨下段骨折的疗效比较

Closed reduction and interlocking intramedullary nailing in the treatment of distal tibial fractures in different age groups

目的探究闭合及切开复位交锁髓内钉内固定在不同年龄段胫骨下段骨折患者治疗中的临床效果。方法选择德兴市人民医院2018年6月—2020年4月收治的胫骨下段骨折患者60例,依据年龄段不同分为青年组、中年组及老年组,每组各20例;另依据不同的治疗方法将3组患者分别分为开放复位组及闭合复位组,每组10例,闭合复位组采用闭合复位交锁髓内钉内固定治疗,开放复位组采用切开复位交锁髓内钉内固定治疗。对比不同年龄阶段及不同手术方法下胫骨下段骨折患者的治疗效果及并发症发生率。结果青年组治疗有效率高于中年组及老年组,闭合复位组治疗有效率高于开放复位组,有统计学差异(P<0.05);青年组患者术后并发症发生率低于中年组及老年组,闭合复位组术后并发症发生率低于开放复位组,有统计学差异(P<0.05)。结论青年胫骨下段骨折患者给予闭合复位交锁髓内钉内固定治疗疗效确切,能够恢复各关节功能,术后并发症少,安全性高,值得推广。

Objective To investigate the clinical effect of closed reduction and interlocking intramedullary nail internal fixation in the treatment of lower tibial fractures in different age groups.Methods Sixty patients with lower tibial fracture admitted to our hospital from June 2018 to April 2020 were selected and divided into young group and middle-aged group according to different age groups,with 20 cases in each group.In addition,the three groups were divided into open reduction and closed reduction group according to different treatment methods,with 10 cases in each group.The closed reduction group was treated with closed reduction and interlocking intramedullary nail internal fixation,and the open reduction group was treated with open reduction and interlocking intramedullary nail internal fixation.To compare the therapeutic effect and the incidence rate of complications in patients with lower tibial fractures at different ages and under different surgical methods.Results The effective rate of treatment in the young group was higher than that in the middle-aged group,and the effective rate in the closed reduction group was higher than that in the open reduction group(P<0.05).The incidence of postoperative complications in the young group was lower than that in the middle-aged group,and the incidence of postoperative complications in the closed reduction group was lower than that in the open reduction group(P<0.05).Conclusion Closed reduction and interlocking intramedullary nail is effective in the treatment of young patients with lower tibial fractures.It can restore the function of each joint,with less postoperative complications and high safety.It is worthy of being widely popularized and applied.
