止锁扩弓非拔牙矫治技术治疗重度牙列拥挤1例并文献复习 被引量:6

CLINICAL EFFECT OF THE NON-EXTRACTION ORTHODONTIC TECHNIQUE OF LOCKING AND ARCH EXPANSION IN TREATMENT OF SEVERE CROWDING:A CASE REPORT AND LITERATURE REVIEW

摘要 目的探讨球面自锁托槽止锁扩弓非拔牙矫治技术治疗重度牙列拥挤的效果。方法安氏Ⅰ类错[牙合]重度牙列拥挤患者1例,应用球面自锁托槽止锁扩弓非拔牙矫治技术进行扩弓,测量矫治前后上下颌牙弓宽度、下颌3、4、5、6牙位的冠中心(FA)点距离、下颌WALA嵴宽度以及下颌4、5、6牙位FA点-WALA嵴距离。结果矫治7个月时,患者牙列拥挤解除,牙弓形态良好。上下颌牙弓宽度较矫治前均有所增加,尤其4-4牙位间宽度增加最为明显;FA点距离及WALA嵴宽度均增加,FA点-WALA嵴距离接近正常值;前牙唇倾量不大。结论球面自锁托槽止锁扩弓非拔牙矫治技术可有效解除重度牙列拥挤,排齐牙列。 Objective To investigate the clinical effect of the non-extraction orthodontic technique of locking and arch expansion with a spherical self-ligating bracket in the treatment of severe crowding.Methods A patient,diagnosed with Angle ClassⅠmalocclusion and severe crowding,was treated with the non-extraction locking and arch expansion technique with a spherical self-ligating bracket.Related parameters were measured before and after treatment,including the width of the maxillary and mandibular dental arches,the distance of facial-axis(FA)points for tooth positions 3,4,5,and 6 of the mandible,the width of mandibular WALA ridge,and the distance between FA point and WALA ridge for tooth positions 4,5,and 6 of the mandible.Results\At month 7 of treatment,dental crowding was relieved,and the dental arch showed a good shape.There were increases in the width of the maxillary and mandibular dental arches after treatment,especially in the 4-4 tooth position;there were also increases in the distance of FA points and the width of WALA ridge,and the distance between FA point and WALA ridge was close to the normal value;there was only slight labial inclination of the anterior teeth.Conclusion The non-extraction locking and arch expansion technique with a spherical self-ligating bracket can effectively correct severe dental crowding and help to achieve well-aligned dentition.
