血清NT-pro-BNP与乳酸在脓毒症患者中的表达及相关性研究

The Expression and Correlation Analysis of Serum N-terminal Probrain Natriuretic Peptide and Lactic Acid in Patients with Sepsis

目的探讨血清N-末端脑钠肽前体(NT-pro-BNP)和乳酸在不同严重程度脓毒症患者中的表达水平及相关性.方法回顾性分析89例脓毒症患者和32例同期非脓毒症住院患者(对照组)的临床资料,脓毒症患者按病情严重程度分为3组:脓毒症组(26例);严重脓毒症组(30例);脓毒症休克组(33例).研究对象入组6h内,记录血清NT-pro-BNP及乳酸表达水平并采用Pearson相关性分析评估二者相关性.结果①脓毒症组血清NT-pro-BNP、乳酸水平显著高于对照组(P<0.01);随脓毒症严重程度增加,血清NT-pro-BNP、乳酸水平显著增加(P<0.01);②脓毒症组患者血清NT-pro-BNP和乳酸表达水平呈正相关(r=0.7219,P<0.01),严重脓毒症组患者血清NT-pro-BNP和乳酸表达水平呈正相关(r=0.7867,P<0.01),脓毒症休克组患者血清NT-pro-BNP和乳酸表达水平也呈正相关(r=0.7985,P<0.01).结论血清NT-pro-BNP及乳酸表达水平随疾病严重程度增加而升高,二者表达水平呈正相关.血清NT-pro-BNP和乳酸水平对评估脓毒症患者病情严重程度具有重要临床应用价值.

Objective To explore the expression level and correlation of serum N-terminal probrain natriuretic peptide(NT-pro-BNP)and lactic acid in patients with sepsis of different se-verity.Methods A total of 89 patients with sepsis and 32 patients without sepsis(control group)were reviewed.Patients with sepsis were divided into three groups according to the severity of sepsis:sepsis group(n=26),severe sepsis group(n=30)and septic shock group(n=33).Within 6 hours of enrollment of the subjects,the expression levels of serum NT-pro-BNP and lactic acid were recorded and the correlation between the two was assessed by Pearson corre-lation analysis method.Results Serum NT-pro-BNP and lactic acid levels in the sepsis group were significantly higher than those in the control group(P<0.01).As the severity of sepsis in-creases,the levels of serum NT-pro-BNP and lactic acid increased significantly(P<0.01).Ser-um NT-pro-BNP and lactic acid expression levels in patients with sepsis were positively corre-lated(r=0.7219,P<0.01),and serum NT-pro-BNP and lactic acid expression levels in patients with severe sepsis were positively correlated(r=0.7867,P<0.01).The expression levels of ser-um NT-pro-BNP and lactic acid in patients with septic shock were also positively correlated(r=0.7985,P<0.01).Conclusion The expression levels of serum NT-pro-BNP and lactic acid in-creased with the severity of the disease,and the expression levels of the two were positively correlated.Serum NT-pro-BNP and lactic acid levels have important clinical application value for assessing the severity of sepsis patients.
