慢性阻塞性肺疾病合并肺曲霉菌感染与肺功能及PTX3基因单核苷酸多态性的关联性 被引量:6

Association of pulmonary Aspergillus infection in COPD patients with lung function single nucleotide polymorphism of PTX3 gene

摘要 目的探讨慢性阻塞性肺疾病(COPD)合并肺曲霉菌感染与肺功能及正五聚蛋白-3(PTX3)基因单核苷酸多态性的关联性。方法选择武警江西总队医院2019年1月-2020年5月COPD合并肺曲霉菌感染患者60例为研究组,选择同期医院收治COPD未合并肺部感染患者80例为对照组。采用肺功能检测仪检测两组患者肺功能指标第1秒钟用力呼吸容积(FEV1)水平;采用限制性片段长度多态性聚合酶链法检测PTX3基因rs1840680位点多态性;采用酶联免疫吸附法检测两组患者血清PTX3水平,分析PTX3基因rs1840680位点多态性与COPD患者并发肺曲霉菌感染的易感性及与肺功能的关系。结果研究组患者PTX3基因rs1840680位点AA基因型频率、A等位基因频率(21.67%、43.33%)高于对照组,差异具有统计学意义(P<0.05),两组GG、GA基因型频率比较,无统计学差异;Logistic回归分析结果显示携带AA基因型、A等位基因为COPD并发肺曲霉菌感染的独立危险因素(P<0.05);慢性阻塞性肺疾病全球倡议(GOLD)分级Ⅰ-Ⅱ级与GOLD分级Ⅲ-Ⅳ级的研究组患者PTX3基因rs1840680位点基因型分布比较,无统计学差异,GOLD分级Ⅰ-Ⅱ级研究组患者血清PTX3水平(79.94±19.34)ng/ml低于GOLD分级Ⅲ-Ⅳ级患者(111.82±28.62)ng/ml,差异具有统计学意义(P<0.05)。结论PTX3基因rs1840680位点多态性与本地区COPD患者并发肺曲霉菌感染有关,A等位基因为易感基因,但与肺功能无关。 OBJECTIVE To explore the association of pulmonary Aspergillus infection in chronic obstructive pulmonary disease(COPD)patients with lung function single nucleotide polymorphism of pentraxin-3(PTX3)gene.METHODS A total of 60 COPD patients complicated with pulmonary Aspergillus infection who were treated in Jiangxi Armed Police Corps Hospital from Jan 2019 to May 2020 were chosen as the study group,meanwhile,80 COPD patients who were not complicated with pulmonary infection were assigned as the control group.The lung function index forced expiratory volume in 1 second(FEV1)was detected by using lung function tester,the polymorphism of PTX3 gene at rs1840680 locus was detected by using restriction fragment length polymorphism polymerase chain method,the serum PTX3 level was determined by means of enzyme-linked immunosorbent assay,and the correlation between the polymorphism of PTX3 gene at rs1840680 locus and the susceptibility to pulmonary Aspergillus infection and lung function of the COPD patients was observed.RESULTS The frequencies of genotypes AA at rs1840680 locus of PTX3 gene and allele A of the study group were respectively,21.67%and 43.33%,significantly higher than those of the control group(P<0.05);there was no significant difference in the frequency of genotype GG,GA between the two groups.Logistic regression analysis showed that the genotype AA and allele A were the independent risk factors for the pulmonary Aspergillus infection in the COPD patients(P<0.05).In the study group,there was no significant difference in the distribution of genotypes at rs1840680 locus of PTX3 gene between the patients with global initiative for chronic obstructive pulmonary disease(GOLD)ranging fromⅠtoⅡand the patients with GOLD ranging fromⅢtoⅣ.The serum PTX3 level of the patients with GOLD ranging fromⅠtoⅡwas(79.94±19.34)ng/ml,significantly lower than(111.82±28.62)ng/ml of the patients with GOLD ranging fromⅢtoⅣ(P<0.05).CONCLUSION The polymorphism at rs1840680 locus of PTX3 gene is associated with the pulmonary Aspergillus infection in the COPD patients,and the allele A is a susceptible gene but was not associated with the lung function.
