胃窗超声造影、MSCT及两者联合诊断胃癌术前TNM分期与手术后病理一致性研究

Study on the Pathological Consistency between Gastric Window Ultrasound Imaging,MSCT,and Both Preoperative TNM Staging and Postoperative Diagnosis of Gastric Cancer

目的分析胃窗超声造影、MSCT及两者联合诊断胃癌术前TNM分期与手术后病理一致性。方法选取本院2017年4月至2019年6月收治的75例胃癌患者,在所有患者在未接受手术及其他治疗前使用胃窗超声造影、MSCT检查。对患者所得图像进行分析,以病理检查结果为“金标准”,比较胃窗超声造影、MSCT及两者联合诊断胃癌术前TNM分期与手术后病理符合率。结果胃窗超声造影、MSCT及两者联合检查对胃癌术前T分期诊断准确率分别为81.33%、78.66%、93.33%,胃窗超声造影、MSCT及两者联合检查对胃癌术前N分期诊断准确率分别为70.66%、80.00%、93.33%,联合检查对胃癌术前T、N分期明显高于单独检查(P<0.05);胃窗超声造影、MSCT及两者联合检查对胃癌术前M分期诊断准确率分别为94.66%、96.00%、98.66%,联合检查稍高于单独检查,但比较无差异(P>0.05)。结论胃窗超声造影、MSCT均为胃癌TNM分期的有效检查手段,但两者联合诊断胃癌术前TNM分期诊断准确性更佳,与术后病理结果一致性较高。

Objective To analyze the pathological consistency between preoperative TNM staging and postoperative diagnosis of gastric window contrast-enhanced ultrasound,MSCT and the combination of them of gastric cancer.Methods 75 patients with gastric cancer treated in our hospital from April 2017 to June 2019 were selected.All patients were examined with gastric window contrast-enhanced ultrasound and MSCT before treated with surgery and other treatments.The images of the patients were analyzed,and the result of pathological examination were used as the"gold standard",and the coincidence rate of gastric window contrast-enhanced ultrasound,MSCT and the combination of them in preoperative TNM staging and postoperative pathological diagnosis of gastric cancer were compared.Results The diagnostic accuracy of gastric window contrast-enhanced ultrasound,MSCT and the combination of them in the diagnosis of preoperative T staging of gastric cancer was 81.33%,78.66%,and 93.33%,respectively.The accuracy rates of oral gastric window contrast-enhanced ultrasound,MSCT and the combination of them in the diagnosis of preoperative N staging of gastric cancer was 70.66%,80.00%,and 93.33%,respectively.The combined examination for preoperative T and N staging of gastric cancer was significantly higher than that of the single examination(P<0.05).The diagnostic accuracy of gastric window contrast-enhanced ultrasound,MSCT and the combination of gastric cancer for preoperative M staging was 94.66%,96.00%,and 98.66%,respectively.the combined examination was slightly higher than that of the single examination,but there was no difference(P>0.05).Conclusion The gastric window contrast-enhanced ultrasound,and MSCT are both effective methods in the diagnosis of preoperative TNM staging of gastric cancer,but the combination of them in the diagnosis of gastric cancer is more accurate and consistent with the postoperative pathological results.
