玻璃纤维桩联合树脂修复对恒牙冠折患者患齿脱落率及美学效果的影响

Effect of fiberglass post combined with resin restoration on tooth loss rate and aesthetic effect in patients with crown fracture of permanent teeth

目的探讨玻璃纤维桩联合树脂修复对恒牙冠折患者患齿脱落率及美学效果的影响。方法选取2019年1月—2020年6月本院就诊的恒牙冠折患者82例,根据随机数字表法分为对照组和研究组,每组41例。对照组采用单独树脂修复,研究组采用树脂联合玻璃纤维桩修复,比较两组患者咀嚼效能,龈沟液前列腺素E2(PGE2)、白细胞介素-1β(IL-1β)水平,比较患齿脱落率及美学效果。结果治疗前2组咬合力和咀嚼效率比较,差异无统计学意义(P>0.05);与治疗前比较,治疗后2组咬合力和咀嚼效率明显升高(P<0.05),且研究组明显高于对照组(P<0.05)。治疗前2组龈沟液PGE2、IL-1β水平比较,差异无统计学意义(P>0.05);与治疗前比较,治疗后龈沟液PGE2、IL-1β水平明显升高,研究组低于对照组,差异有统计学意义(P<0.05)。对照组患齿脱落率为21.95%,研究组为2.44%;研究组低于对照组,差异有统计学意义(P<0.05)。治疗后研究组固位、颜色、外观评分等美学效果均高于对照组,差异有统计学意义(P<0.05)。结论玻璃纤维桩联合树脂用于恒牙冠折修复效果优于树脂修复,降低患齿脱落率,具有一定的美学效果,改善患者咀嚼功能和咬合力,减少牙周组织损伤。

Objective To investigate the effect of glass fiber post combined with resin restoration on tooth loss rate and aesthetic effect in patients with crown fracture of permanent teeth.Methods From January 2019 to June 2020,82 patients with crown fracture of permanent teeth in our hospital were selected and divided into control group and study group according to random number table method,with 41 patients in each group.The control group was treated with resin alone,and the study group was treated with resin combined with glass fiber post.The masticatory efficiency,the levels of prostagstagin E2(PGE2)and interleukin-1β(IL-1β)in gingival crevicular fluid were compared between the two groups,and the rate of tooth loss and aesthetic effect were compared.Results There was no significant difference in bite force and chewing efficiency between the two groups before treatment(P>0.05).Compared with before treatment,the bite force and chewing efficiency of the two groups were significantly increased after treatment(P<0.05),and the study group was significantly higher than the control group(P<0.05).Before treatment,there was no significant difference in the levels of PGE2 and IL-1βin gingival crevicular fluid between the two groups(P>0.05).Compared with before treatment,the levels of PGE2 and IL-1βin gingival crevular fluid in study group were significantly higher than those in control group,and the difference was statistically significant(P<0.05).The rate of tooth loss was 21.95%in the control group and 2.44%in the study group.The study group was lower than the control group,the difference was statistically significant(P<0.05).After treatment,the aesthetic effects such as retention,color and appearance score in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion The effect of glass fiber post combine with resin in crown fracture restoration of permanent teeth is better than that of resin restoration,which can reduce the rate of tooth loss and has certain aesthetic effect,improve the patients'chewing function and bite force,and reduce periodontal tissue damage.
