非小细胞肺癌组织中miR-454-3p、CPEB1 mRNA表达水平及临床意义 被引量:2

摘要 目的分析非小细胞肺癌组织中微小RNA-454-3p(miR-454-3p)、胞质多聚腺苷酸化成分结合蛋白1(CPEB1)信使RNA(mRNA)表达水平与临床病理特征及预后的相关性。方法选取2014年4月—2016年3月在中国科学院合肥肿瘤医院确诊为非小细胞肺癌患者96例,术中取其癌组织设为肺癌组,癌旁正常组织为对照组。采用实时荧光定量PCR法检测2组miR-454-3p、CPEB1 mRNA表达水平;以二者表达水平均值为界,再分为miR-454-3p低表达亚组、高表达亚组和CPEB1 mRNA低表达亚组、高表达亚组,分析癌组织中miR-454-3p、CPEB1 mRNA表达水平与患者临床病理特征及预后的关系;Logistic回归分析影响非小细胞肺癌患者预后的因素。结果肺癌组miR-454-3p表达水平高于对照组,CPEB1 mRNA表达水平低于对照组(t/P=14.130/0.000、12.541/0.000)。患者癌组织miR-454-3p、CPEB1 mRNA表达水平与性别、年龄、吸烟史、分化程度无关(P>0.05),与肿瘤直径、病理类型、淋巴结远处转移、TNM分期有关(miR-454-3p:χ^(2)/P=7.102/0.008、13.577/0.000、9.687/0.002、14.291/0.000;CPEB1 mRNA:χ^(2)/P=10.816/0.001、18.288/0.000、14.122/0.000、16.239/0.000);癌组织miR-454-3p与CPEB1 mRNA表达水平呈负相关(r/P=-0.524/0.000)。miR-454-3p高表达患者5年生存率低于miR-454-3p低表达患者(χ^(2)/P=7.012/0.008),CPEB1 mRNA高表达患者5年生存率高于CPEB1 mRNA低表达患者(χ^(2)/P=4.112/0.043);miR-454-3p高表达、淋巴结远处转移是影响非小细胞肺癌患者死亡的独立危险因素[OR(95%CI)=2.255(1.725~2.948)、1.987(1.428~2.765)],CPEB1 mRNA高表达是影响患者死亡的保护因素[OR(95%CI)=0.677(0.518~0.886)]。结论非小细胞肺癌组织中miR-454-3p呈高表达,CPEB1 mRNA呈低表达,且二者均与患者肿瘤直径、病理类型、淋巴结远处转移、TNM分期及预后有关。 Objective To analyze the correlation between the expression levels of microRNA-454-3p(miR-454-3p)and cytoplasmic polyadenylation element binding protein 1(CPEB1)messenger RNA(mRNA)in non-small cell lung cancer tissues and clinicopathological features and prognosis.Methods From April 2014 to March 2016,96 patients with non-small cell lung cancer tissues diagnosed by chest imaging and histopathology in Hefei Cancer Hospital of Chinese Academy of Sciences were selected,the cancerous tissues were set as lung cancer group and the adjacent normal tissues as control group.Real-time fluorescent quantitative PCR method was used to detect the expression levels of miR-454-3p and CPEB1 mRNA in the two groups;taking the average of the two expression levels as the boundary,the patients were divided into the miR-454-3p low expression subgroup,the miR-454-3p high expression subgroup,and the CPEB1 mRNA low expression subgroup,and the CPEB1 mRNA high expression subgroup,the relationship between the expression levels of miR-454-3p and CPEB1 mRNA in cancer tissues and the clinicopathological characteristics and prognosis of lung cancer patients and the factors influencing the prognosis of patients with non-small cell lung cancer was analyzed.Results The expression level of miR-454-3p in the lung cancer group was higher than that in the control group,and the expression level of CPEB1 mRNA was lower than that in the control group(t/P=14.130/0.000,12.541/0.000);the expression levels of miR-454-3p and CPEB1 mRNA in cancer tissue of patients were not related to gender,age,smoking history and differentiation degree(P>0.05),but related to tumor diameter,pathological type,lymph node distant metastasis and TNM stage(miR-454-3pχ^(2)/P=7.102/0.008,13.577/0.000,9.687/0.002,14.122/0.000,CPEB1 mRNA:10.816/0.001,18.288/0.000,14.291/0.000,16.239/0.000);there was a negative correlation between miR-454-3p and CPEB1 mRNA expression in cancer tissues of patients with lung cancer(r/P=-0.524/0.000);the 5-year survival rate of patients with high expression of miR-454-3p was lower than that of patients with low expression of miR-454-3p,the 5-year survival rate of patients with high expression of CPEB1 mRNA was higher than that of patients with low expression of CPEB1 mRNA(χ^(2)/P=7.012/0.008,4.112/0.043);miR-454-3p and distant lymph node metastasis were independent risk factors for death in patients with non-small cell lung cancer tissues[OR(95%CI)=2.255(1.725-2.948),1.987(1.428-2.765)],CPEB1 mRNA was protective factor for death in patients with non-small cell lung cancer tissues[OR(95%CI)=0.677(0.518-0.886)].Conclusion MiR-454-3p is highly expressed in non-small cell lung cancer tissues and CPEB1 mRNA is low expressed,and both of them are related to tumor diameter,pathological type,distant lymph node metastasis,TNM staging and prognosis in patients with lung cancer.
