根管治疗牙体牙髓病200例临床分析

Root Canal Therapy for Endodontic Disease:a Clinical Analysis of 200 Cases

目的探讨牙体牙髓病根管治疗中根管治疗次数、根管填充量对治疗效果的影响。方法对新干县中医医院2019年3月至2020年6月收治的200例牙体牙髓病患者的临床资料进行回顾性分析,均行根管治疗。其中多次法根管治疗91例,一次性根管治疗109例;根管适充治疗112例,超充治疗88例。结果200例牙体牙髓病患者经治疗后,显效109例,有效75例,无效16例,临床治疗总有效率为92.00%。多次法根管治疗和一次性根管治疗总有效率分别为90.11%、93.58%,二者比较差异无统计学意义(P>0.05);但咀嚼功能、语言功能和美观度三项指标评分比较,一次性根管治疗显著高于多次法根管治疗,差异均有统计学意义(P<0.05)。根管适充治疗和超充治疗总有效率分别为96.43%、86.36%,适充治疗显著高于超充治疗,差异有统计学意义(P<0.05);同时适充治疗咀嚼功能、语言功能和美观度评分均显著高于超充治疗组,差异均有统计学意义(P<0.05)。结论对牙体牙髓病的治疗,选择一次性根管治疗,并在根管填充时选择适充疗法,可获得更好的临床疗效,同时有利于患者咀嚼功能、语言功能的恢复,满足患者的美观需求。

Objective To investigate the effect of the number of root canal treatments and the amount of root canal filling on endodontic treatment of endodontic diseases.Methods Clinical data of 200 patients who underwent root canal therapy for endodontic disease in Xingan County Hospital of Traditional Chinese Medicine between March 2019 and June 2020 were analyzed retrospectively.Among them,91 patients received multiple-visit root canal treatment,109 patients received single-visit root canal treatment,112 patients received appropriate filling,and 88 patients received excessive filling.Results The treatment was significantly effective in 109 cases,moderately effective in 75 cases,and ineffective in 16 cases,with a total effective rate of 92.00%.There was no significant difference in the clinically effective rate between multiple-visit root canal treatment(90.11%)and single-visit root canal treatment(93.58%)(P>0.05).However,single-visit root canal treatment achieved higher scores of chewing function,language function and aesthetic degree than multiple-visit root canal treatment(P<0.05).The effective rate of appropriate filling treatment(96.43%)was higher than that of excessive filling treatment(86.36%)(P<0.05).In addition,compared with excessive filling treatment,appropriate filling treatment significantly increased the scores of chewing function,language function and aesthetic degree(P<0.05).Conclusion Single-visit root canal treatment with appropriate filling is effective for endodontic disease,and is beneficial to the recovery of masticatory and language functions and the aesthetic outcomes.
