腹膜透析患者基线腹膜转运特征及高转运的影响因素 被引量:1

摘要 目的探讨腹膜透析患者基线腹膜转运的特征,分析腹膜高转运的影响因素。方法回顾性分析2016年1月至2018年9月在新乡医学院第一附属医院肾内科首次接受腹膜透析的102例患者作为研究对象。根据首次腹膜平衡实验(PET)结果,将患者分为高转运组(高转运组及高平均转运组)和低转运组(低转运组及低平均转运组),比较两组人口学、临床生化学指标,采用多因素logistic回归分析腹膜高转运的影响因素。结果所有腹膜透析患者的平均D/Pcr为0.69±0.14,高转运组患者男性比率较高,体表面积较大,白蛋白、血红蛋白、血清氯离子水平较低。多因素logistic回归分析显示,腹膜高转运特性的保护性因素有3个:白蛋白(OR=0.893,95%CI:0.807~0.989,P=0.029)、血红蛋白(OR=0.969,95%CI:0.944~0.996,P=0.024)和血清氯离子(OR=0.874,95%CI:0.774~0.987,P=0.030)。结论腹膜透析患者基线腹膜转运功能与性别、体表面积、白蛋白、血红蛋白和血清氯离子水平有关,血清白蛋白、血红蛋白和血清氯离子水平是腹膜高转运的保护性因素。 Objective To investigate the baseline peritoneal transport characteristics and influence factors of high transport in peritoneal dialysis(PD)patients.Methods A total of 102 PD patients who firstly accepted PD from January 2016 to September 2018 in the First Affiliated Hospital of Xinxiang Medical College were collected.The patients were divided into high-transport group(high and high-average)and low-transport group(low-average and low)according to the first peritoneal equilibration test(PET)result.The demographics and clinical biochemical indexes were compared between the two groups.Multivariate logistic regression was used to analyze the influencing factors of high peritoneal transport.Results The average D/Pcr value of all PD patients was 0.69±0.14.Compared with low-transport group,the proportion of male patients of high-transport group was higher,the body surface area was larger,the levels of hemoglobin,serum chloride and serum albumin were lower.Multivariate logistic regression analysis showed that serum albumin(OR=0.893,95%CI:0.807-0.989,P=0.029),Hemoglobin(OR=0.969,95%CI:0.944-0.996,P=0.024)and serum chloride level(OR=0.874,95%CI:0.774-0.987,P=0.030)were the independent protective factors for high peritoneal transport characteristics.Conclusion The baseline peritoneal transport function in PD patients was related to gender,body surface area,albumin,hemoglobin and serum chloride level.Serum albumin,hemoglobin and serum chloride levels were the protective factors of high peritoneal transport.
