超声监视下宫腔镜电切术治疗黏膜下子宫肌瘤的效果及安全性分析

Effects and Safety of Hysteroscopic Resection Under Ultrasound Monitoring in Patients with Submucosal Uterine Fibroids

目的探讨超声监视下宫腔镜电切术对黏膜下子宫肌瘤患者的治疗效果及安全性。方法选取2017年12月至2019年11月汝阳县妇幼保健院收治的86例黏膜下子宫肌瘤患者作为研究对象,根据手术方案分为超声组和常规组两组,各43例。超声组接受超声监视下宫腔镜电切术,常规组接受常规宫腔镜电切术。比较两组手术相关指标(术中出血量、住院时间、手术时间、下床时间、肛门排气时间)、并发症发生情况,同时依据视觉模拟量表(VAS)评估患者术后1、3、5 d疼痛情况,分析超声监视下宫腔镜电切术对于黏膜下子宫肌瘤患者的治疗效果及安全性。结果超声组术中出血量少于常规组,住院时间、手术时间、下床时间、肛门排气时间短于常规组,并发症发生率低于常规组,术后1、3、5 d VAS评分低于常规组,差异有统计学意义(P<0.05)。结论黏膜下子宫肌瘤患者接受超声监视下宫腔镜电切术治疗有利于缩短手术时间,减轻机体损伤,缓解手术疼痛,降低并发症发生风险,促进患者康复。

Objective To investigate the therapeutic effects and safety of hysteroscopic resection under ultrasound monitoring in patients with submucosal uterine fibroids.Methods A total of 86 patients with submucosal uterine fibroids who were treated in Ruyang Maternity and Child Health Hospital from December 2017 to November 2019 were collected and divided into two groups according to surgical plans,with 43 cases in each group.The ultrasound group accepted hysteroscopic resection by use of ultrasound surveillance,and the conventional group accepted conventional hysteroscopic resection.The operation-related indicators(intraoperative blood loss,hospitalization time,operation time,time to get out of bed and anal exhaust time),the occurrence of complications and the scores of visual analog scale(VAS)at 1,3,and 5 days after operation were compared.Results The intraoperative blood loss of the ultrasound group was less than that of the conventional group(P<0.05).The hospitalization time,operation time,time to get out of bed and anal exhaust time of the ultrasound group were shorter than those of the conventional group(P<0.05).The incidence of complications of the ultrasound group was lower than that of the conventional group(P<0.05).The VAS scores at 1,3,5 days after operation of the ultrasound group were significantly lower than those of the conventional group(P<0.05).Conclusion The hysteroscopic resection under ultrasound monitoring is beneficial to shorten the operation time of patients with submucosal uterine fibroids,reduce body damage,relieve surgical pain,reduce the risk of complications and promote recovery.
