分阶段健康教育联合舒适护理对妊娠糖尿病患者分娩结局和血糖、血脂的影响

Effects of Staged Health Education Combined with Comfortable Nursing on Delivery Outcome,Blood Glucose and Lipid Levels in Patients with Gestational Diabetes Mellitus

目的分析分阶段健康教育联合舒适护理对妊娠糖尿病(GDM)患者分娩结局和血糖、血脂的影响。方法收集2019年6月至2020年6月郑州大学人民医院产科收治的80例妊娠糖尿病患者作为研究对象,随机分为试验组和对照组,各40例。对照组接受常规护理,试验组在常规护理的基础上接受分阶段健康教育联合舒适护理。比较两组患者护理前及护理1、2个月后的血糖、血脂水平,分娩结局以及分娩方式。结果护理前,两组空腹血糖、餐后2 h血糖、糖化血红蛋白水平比较,差异无统计学意义(P>0.05);护理1、2个月后,空腹血糖、餐后2 h血糖、糖化血红蛋白水平均低于护理前(P<0.05),试验组空腹血糖、餐后2 h血糖、糖化血红蛋白水平均低于对照组(P<0.05)。护理前,两组甘油三酯、总胆固醇水平比较,差异无统计学意义(P>0.05);护理1、2个月后的甘油三酯、总胆固醇水平均低于护理前(P<0.05),试验组患者的甘油三酯、总胆固醇水平均低于对照组(P<0.05)。试验组患者不良分娩结局的总发生率低于对照组(P<0.05)。结论分阶段健康教育联合舒适护理能够改善GDM患者的血糖、血脂水平,降低不良分娩结局的发生率,提高自然分娩率,在妇产科临床护理中值得借鉴。

Objective To analyze the effects of staged health education combined with comfort nursing on delivery outcome,blood glucose and lipid levels in patients with gestational diabetes mellitus(GDM).Methods A total of 80 patients with gestational diabetes mellitus who were treated in Zhengzhou University People’s Hospital from June 2019 to June 2020 were collected and randomly divided into experimental group and control group,with 40 cases in each group.The control group accepted routine nursing intervention,and the experimental group accepted staged health education combined with comfort nursing on the basis of routine nursing.The levels of blood glucose,blood lipid and glycosylated hemoglobin,delivery outcome and delivery mode of the two groups were compared.Results Before nursing,there was no significant difference in the levels of fasting blood glucose(FBG),2-hour postprandial blood glucose(2 h PG)and hemoglobin A1 c(HbA1 c)between the two groups(P>0.05).After 1 and 2 months’nursing,the levels of FBG,2 h PG and HbA1 c of the two groups were decreased,and the levels of FBG,2 h PG and HbA1 c of the experimental group were lower than those of the control group(P<0.05).Before nursing,there was no significant difference in the levels of triglyceride(TG)and total cholesterol(TC)between the two groups(P>0.05).After 1 and 2 months’nursing,the levels of TG and TC of the two groups were decreased,and the levels of TG and TC of the experimental group were lower than those of the control group(P<0.05).The total incidence of adverse delivery outcomes of the experimental group was lower than that of the control group(P<0.05).Conclusion Staged health education combined with comfort nursing can significantly improve the levels of blood glucose and blood lipid of GDM patients,reduce the incidence of adverse delivery outcomes and improve the natural delivery rate,which is worthy of reference in the clinical nursing of obstetrics and gynecology.
