温化方联合经肝动脉化疗栓塞术治疗中晚期肝癌患者的临床疗效分析

Clinical analysis of"Wen Hua Fang"decoction combined with TACE in patients with advanced liver cancer

目的:探讨温化方联合经肝动脉化疗栓塞术(TACE)治疗中晚期肝癌患者的临床疗效。方法:选取2016年10月至2019年9月我科收治的中晚期肝癌患者62例,按照系统抽样方法随机分成对照组(31例)和观察组(31例)。对照组患者予TACE治疗,观察组患者在对照组的基础上加服温化方汤剂。治疗3个月后,比较两组患者的肝功能、肿瘤标记物水平、中医临床证候改善率、临床疗效以及生活质量。结果:①治疗后观察组患者的血清谷草转氨酶(AST)、谷丙转氨酶(ALT)、总胆红素(TBil)以及白蛋白(Alb)水平均显著低于对照组(P<0.05);②治疗后观察组患者的血清甲胎蛋白(AFP)、糖类抗原CA19-9(CA19-9)水平明显低于对照组(P<0.05);③治疗后观察组患者的中医临床证候改善率为83.87%、有效率为93.55%,显著高于对照组(P<0.05);④观察组患者的各症状缓解情况明显优于对照组(P<0.05);⑤观察组患者的卡氏评分(KPS)、癌症患者生活质量测定量表(QOL)评分在治疗后均显著高于对照组(P<0.05)。结论:TACE联合温化方治疗中晚期肝癌患者,能够有效改善患者肝功能及临床症状,增强治疗效果,并提高患者生活质量。

Objective:To explore the clinical effect of"Wen Hua Fang"decoction combined with TACE in patients with advanced liver cancer.Methods:From October 2016 to September 2019,sixty-two patients with advanced liver cancer were randomly divided into the control group(31 cases)and observation group(31 cases).The patients in the control group were treated with TACE,while those in the observation group were treated with"Wen Hua Fang"Chinese herbal decoction combined with TACE.After three months of treatment,the liver function,tumor marker level,the improvement rate of the clinical syndrome of traditional Chinese medicine(TCM),the clinical effect of western medicine and quality of life of the two groups were compared.Results:①The levels of AST,ALT,TBil,and Alb in the observation group were lower than the control group(P<0.05).②The serum levels of AFP and carbohydrate antigen CA19-9 in the observation group were lower than the control group(P<0.05).③After treatment,the improvement rate of TCM symptoms and signs in the observation group(83.87%)and the effective rate of western medicine(93.55%)were significantly higher than those in the control group(P<0.05).④The relief of symptoms in the observation group was better than the control group(P<0.05).⑤The scores of KPS and QOL in the observation group were higher than the control group(P<0.05).Conclusion:On the basis of TACE treatment,it can effectively improve the liver function and clinical symptoms of patients,enhance the treatment effect,and improve the quality of life of patients.It has unique advantages in the treatment of patients with advanced liver cancer and is worth popularizing widely.
