摘要
目的:探讨手术切口感染风险评估量表对开放性手术患者切口愈合及预后的预测价值。方法:选取186例开放性手术患者作为研究对象,统计手术切口感染(SWI)发生率,并对比SWI与无SWI手术切口感染风险评估量表评分,分析美国医院感染监测系统(NNIS)、美国医院感染控制有效性(SENIC)、手术切口感染风险评估量表对切口愈合及预后预测价值。结果:SWI发生率为37.63%(70/186);SWI健康史、术前准备、术中操作、术后护理及总分高于无SWI(P<0.05);结合术后30 d是否发生SWI对手术切口感染风险评估量表、NNIS、SENIC评分绘制ROC曲线,结果显示手术切口感染风险评估量表AUC值0.951、敏感性81.16%、特异性98.14%高于NNIS的0.716、65.84%、71.22%与SENIC的0.603、60.39%、65.28%(P<0.05)。结论:手术切口感染风险评估量表应用于开放性手术患者术后切口愈合与预后评估中,具有高敏感性、特异性,可为临床早期预防提供可靠依据。
Objective:To investigate the predictive value of incision infection risk assessment scale in wound healing and prognosis of patients undergoing open surgery. Methods:186 patients with open surgery were selected as the research objects. The incidence of surgical wound infection(SWI)was calculated,and the risk assessment scale scores of surgical incision infection with SWI and without SWI were compared. Results:The incidence of SWI was 37.63%(70/186). The health history,preoperative preparation,intraoperative operation,postoperative nursing and total score of SWI were higher than those without SWI(P<0.05). The ROC curve was drawn according to whether SWI occurred 30 days after surgery on the risk assessment scale of surgical incision infection,NNIS and SENIC scores.The results showed that the AUC value of the risk assessment scale for surgical incision infection was 0.951,the sensitivity was 81.16%,and the specificity was 98.14%,higher than 0.716,65.84%,71.22% of NNIS and 0.603,60.39%,65.28% of SENIC(P<0.05). Conclusion:The risk assessment scale for surgical wound infection is highly sensitive and specific in the evaluation of postoperative wound healing and prognosis of patients undergoing open surgery,which can provide reliable evidence for early clinical prevention.
