不同预后社区获得性肺炎患儿血清急性时相反应蛋白表达水平的比较

Comparison of Acute Phase Response Protein Expression Levels in Serum of Children with Different Prognosis of Community-acquired Pneumonia

目的:比较不同预后社区获得性肺炎患儿血清急性时相反应蛋白的表达水平。方法:选择2017年8月-2020年5月在本院诊治的144例社区获得性肺炎患儿为研究对象。检测患儿血清急性时相反应蛋白[C反应蛋白(CRP)、结合珠蛋白(HP)、铜蓝蛋白(CP)、α1-酸性糖蛋白(α1-AGP)]与常规生化指标含量。根据住院时间是否≤1周将患儿分为预后不良组(住院时间>1周)与预后良好组(住院时间≤1周),比较两组上述指标检测情况。结果:144例患儿的平均住院时间(4.78±0.31)d,32例住院时间>1周,112例住院时间≤1周。预后良好组的白细胞计数、D-二聚体均显著低于预后不良组,差异均有统计学意义(P<0.05);两组血沉、纤维蛋白原比较,差异均无统计学意义(P>0.05)。预后良好组的血清HP、CP、CRP、α1-AGP含量均显著低于预后不良组,差异均有统计学意义(P<0.05)。结论:血清急性时相反应蛋白在预后良好组中呈现低表达状况,该结果为血清HP、CP、CRP、α1-AGP在社区获得性肺炎患儿预后预测中提供一定的数据参考。

Objective:To compare the expression level of acute phase response protein in serum of children with different prognosis of community-acquired pneumonia.Method:A total of 144 children with community-acquired pneumonia treated in our hospital from August 2017 to May 2020 were selected as the research subjects.Serum levels of acute phase reactive protein[C-reactive protein (CRP),haptoglobin (HP),ceruloplasmin (CP),α1-acidic glycoprotein (α1-AGP)]and conventional biochemical indexes were determined.According to the length of hospital stay ≤1 week,the children were divided into poor prognosis group (hospital stay >1 week) and good prognosis group (hospital stay ≤1 week),the detection of the above indicators were compared between two groups.Result:The average hospitalization time of 144 cases was (4.78±0.31) d,32 cases were >1 week,112 cases were ≤1 week.The white blood cell count and D-dimer in good prognosis group were significantly lower than those in poor prognosis group,the differences were statistically significant (P<0.05).Comparison of ESR and fibrinogen between two groups,there were no statistically significant differences (P>0.05).The serum contents of HP,CP,CRP and α1-AGP in good prognosis group were significantly lower than those in poor prognosis group,the differences were statistically significant (P<0.05).Conclusion:Acute phase response protein expression in serum are low in good prognosis group,which provide a certain data reference for predicting the prognosis of serum HP,CP,CRP and α1-AGP in children with community-acquired pneumonia.
