摘要
目的:探讨颈动脉超声在缺血性脑血管病中的诊断价值。方法:选择2019年5月-2020年7月在本院进行诊治的105例疑似缺血性脑血管病患者为试验组,选择同期100名健康体检人群为对照组。两组均进行颈动脉超声检查。以数字减影血管造影检查结果为金标准,分析颈动脉超声对缺血性脑血管病诊断的敏感度、特异度、准确度,统计颈动脉超声对不同程度颈动脉狭窄的诊断效果,比较试验组确诊患者和对照组阻力指数、脑血流量、平均峰流速、颈动脉内中膜厚度。结果:105例疑似缺血性脑血管病患者中经数字减影血管造影检查有97例确诊。颈动脉超声89例诊断准确,误诊1例,漏诊8例。颈动脉超声诊断缺血性脑血管病灵敏度为91.75%、特异度为87.50%、准确度为91.43%。颈动脉超声对确诊患者狭窄程度的诊断符合情况分别为闭塞93.75%、重度狭窄92.31%、中度狭窄93.55%、轻度狭窄87.50%,总符合率91.75%。试验组确诊患者阻力指数、平均峰流速和颈动脉内中膜厚度均高于对照组,脑血流量低于对照组,差异均有统计学意义(P<0.05)。结论:在缺血性脑血管病中鉴别诊断中,颈动脉超声具有良好的诊断效能,并能准确判定狭窄程度,可作为临床诊断的可靠依据。
Objective:To explore the diagnostic value of carotid ultrasound in ischemic cerebrovascular disease.Method:A total of 105 patients with suspected ischemic cerebrovascular disease treated in our hospital from May 2019 to July 2020 were selected as the experimental group,100 healthy subjects were selected as the control group.Carotid ultrasound were performed in the two groups.The results of DSA were regarded as the gold standard,the sensitivity,specificity,accuracy of carotid ultrasound in the diagnosis of ischemic cerebrovascular disease were analyzed,the diagnostic effect of carotid ultrasound in different degrees of carotid stenosis was analyzed.The resistance index,cerebral blood flow,average peak velocity,and thickness of carotid media-media were compared between the experimental group and the control group.Result:Among the 105 suspected patients with ischemic cerebrovascular disease,97 were confirmed by DSA.89 cases of carotid ultrasound diagnosis accuracy,misdiagnosis in 1 case,missed diagnosis in 8 cases.The sensitivity,specificity and accuracy of carotid ultrasound in the diagnosis of ischemic cerebrovascular disease were 91.75%,87.50% and 91.43%.The diagnostic accuracy of carotid artery ultrasound in patients with diagnosed stenosis was 93.75% occlusion,92.31% severe stenosis,93.55% moderate stenosis,and 87.50% mild stenosis,respectively,and the overall coincidence rate was 91.75%.The resistance index,average peak flow velocity and carotid inner media thickness in the experimental group were higher than those in the control group,while the cerebral blood flow was lower than that in the control group,the differences were statistically significant (P<0.05).Conclusion:In the differential diagnosis of ischemic cerebrovascular disease,carotid ultrasound has a good diagnostic efficiency and can accurately determine the degree of stenosis,which can be used as a reliable basis for clinical diagnosis.
