三七止血汤对人工全膝关节置换术后康复的影响

Effect of Sanqi Zhixue Decoction on Rehabilitation after Total Knee Arthroplasty

目的:分析三七止血汤对人工全膝关节置换术后康复的影响。方法:选取2018年1月-2019年12月在本院行人工全膝关节置换术的70例患者为研究对象,按随机数字表法将患者分为观察组和对照组,每组35例。两组患者均接受常规治疗,对照组给予氨甲环酸氯化钠注射液治疗,并于术后7 d后改为阿司匹林肠溶片持续用药至术后28 d,观察组在对照组基础上给予三七止血汤联合治疗。比较两组治疗效果。结果:术前,两组血红蛋白、CRP、TNF-α、IL-6水平比较,差异均无统计学意义(P>0.05);术后3、7 d,两组血红蛋白水平均较术前显著下降,且观察组均高于对照组,差异均有统计学意义(P<0.05)。术后3 d,两组CRP、TNF-α、IL-6水平均低于术前,且观察组均低于对照组,差异均有统计学意义(P<0.05)。术前,两组HSS、VAS评分比较,差异均无统计学意义(P>0.05);术后14、28 d,两组HSS评分均高于术前,且观察组均高于对照组,差异均有统计学意义(P<0.05)。术后3、7、14 d,两组VAS评分均低于术前,且观察组均低于对照组,差异均有统计学意义(P<0.05)。观察组术后1、2、3 d的肢体肿胀率均低于对照组,差异均有统计学意义(P<0.05)。结论:对人工全膝关节置换术患者给予三七止血汤治疗,可有效改善其膝关节功能,缓解疼痛,降低炎性因子水平及肢体肿胀率,促进快速康复。

Objective:To analyze the effect of Sanqi Zhixue Decoction on rehabilitation after total knee arthroplasty.Method:A total of 70 patients who underwent total knee replacement in our hospital from January 2018 to December 2019 were selected as the study subjects,the patients were divided into observation group and control group by random number table,35 cases in each group.Both groups received conventional treatment,and the control group was given Tranexamic Acid Sodium Chloride Injection,and changed to Aspirin Enteric Coated Tablet 7 d after surgery until 28 d after surgery,the observation group was given combined treatment with Sanqi Zhixue Decoction on the basis of control group.The therapeutic effects of two groups were compared.Result:Before surgery,comparison of hemoglobin,CRP,TNF-α and IL-6 levels between two groups,there were no significant differences (P>0.05).At 3 and 7 d after surgery,the hemoglobin levels in both groups were significantly decreased compared with those before surgery,and the observation group were higher than those of the control group,the differences were statistically significant (P<0.05).At 3 d after surgery,the levels of CRP,TNF-α and IL-6 in two groups were lower than those before surgery,and the observation group were lower than those of the control group,the differences were statistically significant (P<0.05).Before surgery,comparison of HSS and VAS scores between two groups,there were no significant differences (P>0.05).At 14 and 28 d after surgery,HSS scores in both groups were higher than those before surgery,and the observation group were higher than those of the control group,the differences were statistically significant (P<0.05).At 3,7 and 14 d after surgery,the VAS scores of two groups were lower than those before surgery,and the observation group were lower than those of the control group,the differences were statistically significant (P<0.05).The limb swelling rate of the observation group were lower than those of the control group at 1,2 and 3 d after surgery,the differences were statistically significant (P<0.05).Conclusion:Giving Sanqi Zhixue Decoction to patients undergoing total knee arthroplasty can effectively improve their knee function,relieve pain,reduce the level of inflammatory factors and limb swelling rate,and promote rapid rehabilitation.
