3mm切口超声乳化联合非球面人工晶体植入术治疗白内障患者的疗效

Analysis of the efficacy and safety of 3mm incision phacoemulsification+aspheric intraocular lens implantation in the treatment of cataract patients

目的探究3mm切口超声乳化联合非球面人工晶体植入术治疗白内障患者的效果。方法选取我院2017年10月至2019年4月白内障患者92例,根据治疗方案不同分为研究组(n=46)、对照组(n=46),两组均行3mm切口超声乳化,对照组联合球面人工晶体植入术,研究组联合非球面人工晶体植入术,比较两组术后1 d及术后3个月最佳矫正视力水平、术后1个月3 mm瞳孔高阶像差(角膜高阶像差、晶状体高阶像差、全眼高阶像差)、术后并发症发生率。结果术后3个月两组最佳矫正视力水平均高于术后1 d(t_(1)=8.128,P_(1)<0.001;t_(2)=6.192,P_(2)<0.001),但两组比较差异无统计学意义(t=0.562,P=0.576);术后1个月研究组晶状体高阶像差、全眼高阶像差均低于对照组(t_(1)=10.710,P 1<0.001;t_(2)=9.254,P_(2)<0.001),2组角膜高阶像差比较无明显差异(t=1.397,P=0.066);研究组术后并发症发生率6.52%(3/46)与对照组10.87%(5/46)无明显差异(χ^(2)=0.137,P=0.711)。结论3mm切口超声乳化联合非球面人工晶体植入术治疗白内障患者,可有效提高视力,减少高阶像差。

Objective To explore the effect of 3 mm incision phacoemulsification combined with aspheric intraocular lens implantation in the treatment of cataract patients.Methods From October 2017 to April 2019,92 cataract patients in our hospital were selected and divided into study group(n=46)and control group(n=46)according to different treatment plans.Both groups underwent 3 mm incision phacoemulsification,the control group combined with spherical intraocular lens implantation,and the study group combined with aspherical intraocular lens implantation.The best corrected visual acuity at 1 day and 3 months after the operation was compared between the two groups,and the 3 mm pupil high-order aberrations(corneal high-order aberrations,lens high-order aberrations,whole-eye high-order aberrations)1 month after,the incidence of postoperative complications.Results The best corrected visual acuity of the two groups in the last 3 months was higher than 1 day after operation(t_(1)=8.128,P_(1)<0.001;t_(2)=6.192,P_(2)<0.001),but the difference between the two groups was not statistically significant(t=0.562,P=0.576);The high-order lens aberrations and the high-order aberrations of the whole eye in the study group were lower than those in the control group(t_(1)=10.710,P_(1)<0.001;t_(2)=9.254,P_(2)<0.001).The corneas of the two groups There was no significant difference in higher-order aberrations(t=1.397,P=0.066);the postoperative complication rate of the study group was 6.52%(3/46)and the control group 10.87%(5/46)had no significant difference(χ^(2)=0.137,P=0.711).Conclusion 3 mm incision phacoemulsification+aspherical intraocular lens implantation in the treatment of cataract patients can effectively improve vision,reduce high-order aberrations,and has high safety.
