纤维支气管镜吸痰联合肺泡灌洗治疗重症肺部感染的效果 被引量:4

Effect of fiberoptic bronchoscopy sputum suction combined with alveolar lavage in the treatment of severe pulmonary infection

摘要 目的探讨纤维支气管镜吸痰联合肺泡灌洗(FBBL)治疗重症肺部感染的效果。方法选取辽宁省健康产业集团铁煤总医院2018年6月—2020年12月收治的78例重症肺部感染患者作为研究对象,采用随机抽签法分为观察组(39例)和对照组(39例)。对照组实施常规治疗方法,观察组在对照组基础上采用FBBL疗法,比较两组患者动脉血气指标、住院情况及并发症发生率。结果治疗后,观察的组动脉血二氧化碳分压(PaCO_(2))低于对照组,差异有统计学意义(P<0.05);治疗后,观察组的血氧饱和度(SpO_(2))、酸碱度(pH)高于对照组,差异有统计学意义(P<0.05);观察组的症状改善时间、机械通气时间、抗菌药物使用时间、ICU住院时间短于对照组,差异有统计学意义(P<0.05);治疗期间,观察组的并发症总发生率与对照组比较,差异无统计学意义(P>0.05)。结论FBBL疗法作为重症肺部感染患者的辅助治疗措施,可改善动脉血气指标,改善住院情况,并且不会增加并发症风险,患者安全性和有效性能够得到保障。 Objective To explore the effect of fiberoptic bronchoscope suction combined with alveolar lavage(FBBL)in the treatment of severe pulmonary infections.Methods A total of 78 patients with severe pulmonary infection admitted to the Iron Coal General Hospital of Liaoning Health Industry Group from June 2018 to December 2020 were selected as the research samples,they were divided into observation group(39 cases)and control group(39 cases)by random lottery method.The control group performed conventional treatment methods,on this basis,the observation group used FBBL therapy to compare the arterial blood gas indicators,hospitalization status and complication rate of the two groups.Results After treatment,the partial pressure of carbon dioxide in pulse blood(PaCO_(2))of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05);after treatment,the blood oxygen saturation(SpO_(2))and pondus hydrogenii(pH)of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05);the observation group′s symptom improvement time,mechanical ventilation time,antibacterial drug use time,and ICU hospital stay were shorter than those of the control group,the differences were statistically significant(P<0.05);during the treatment period,the total incidence of complicatiens in the observation group compared with the control group,the difference was not statistically significant(P>0.05).Conclusion As an adjuvant treatment for patients with severe pulmonary infection,FBBL therapy can improve arterial blood gas indicators,improve hospitalization,and will not increase the risk of complications,and patient safety and effectiveness can be guaranteed.
