半髋关节置换术围手术期护理及康复护理研究

Study on Perioperative Nursing and Rehabilitation Nursing for Hemi-Hip Replacement

目的:探究半髋关节置换术围手术期护理及康复护理研究。方法:筛选宁陵县人民医院2018年2月-2019年12月收治的74例髋关节置换术患者,根据患者的护理方式不同分为观察组(39例,采用围手术护理+康复护理)和对照组(35例,采用常规护理),比较两组患者的手术情况、康复情况和术后并发症发生率。结果:两组患者干预后在手术时间上差异无统计学意义(P>0.05),而观察组实施干预后的出血量为(123.69±26.80)ml,术中输液量为(1428.22±59.60)ml,术中输血量为(124.16±14.66)ml,与对照组相比较,各项数值显著更优,差异有统计学意义(P<0.05)。观察组优秀30例,良好9例,无较差例数,优良率100.00%,康复效果显著高于对照组,差异有统计学意义(P<0.05)。两组干预后,观察组术后感染0例,人工髋关节脱位2例,下肢深静脉血栓形成0例,并发症发生率5.13%,并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:对半髋关节置换术患者实施围手术期护理和康复护理,可有效改善手术情况,合理指导患者进行术后康复训练,加快痊愈速度,并且避免围手术期产生不良并发症,可有效提高患者预后功能水平,改善其社会生存质量,值得临床推广。

Objective:To explore the perioperative nursing and rehabilitation nursing of hemi-hip replacement.Methods:A total of 74 hip replacement patients admitted to the hospital from February 2018 to December 2019 were screened.The patients were divided into observation group(39 cases,using perioperative care+rehabilitation nursing) and control group(35 cases,using conventional care) according to different nursing methods.The operation status,recovery status and postoperative complications rate of the two groups of patients were compared.Results:After nursing intervention,there was no statistically significant difference in operation time between the two groups(P>0.05).The bleeding volume of the observation group after the intervention was(123.69±26.80) ml,the intraoperative fluid infusion was(1 428.22±59.60) ml,and the intraoperative blood transfusion was(124.16±14.66)ml,which were significantly better than those of the control group,the difference was statistically significant(P<0.05).In the observation group,30 cases were excellent,9 cases were good,and there were no bad cases.The excellent and good rate was 100.00%.The rehabilitation effect of the observation group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).After nursing intervention,there were 0 case of postoperative infection in the observation group,2 cases of artificial hip dislocation,0 case of deep vein thrombosis of the lower limbs,the complication rate was 5.13%,and the complication rate was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion:Perioperative care and rehabilitation care for patients undergoing hemi-hip replacement can effectively improve the operation situation,rationally guide patients to carry out postoperative rehabilitation training,accelerate the recovery rate,and avoid adverse complications during the perioperative period,which can effectively improve the prognosis of patients.It is worthy of clinical promotion to improve its functional level and improve its social quality of life.
