独活寄生汤联合康复训练对KOA患者全膝关节置换术后早期运动功能的影响

Effect of Duhuo Jisheng Decoction Combined with Rehabilitation Training on Early-Stage Motor Function after Total Knee Arthroplasty

目的:观察独活寄生汤联合康复训练对膝骨关节炎(knee osteoarthritis,KOA)患者全膝关节置换术(total knee arthro-plasty,TKA)后早期运动功能的影响。方法:90例KOA患者按照随机数字表法将患者分为对照组与治疗组,每组各45例。两组患者均行单侧膝关节TKA,对照组术后每日给予常规康复功能训练;治疗组在对照组治疗基础上给予独活寄生汤,两组均治疗14 d。治疗后,比较两组患者术前、术后14 d的视觉模拟评分(visual analogue scale,VAS)、膝关节活动度(range of motion,ROM)、美国特种外科医院(hospital for special surgery,HSS)功能评分、起立-行走计时测试(the time up and go test,TUGT)以及6分钟步行试验(6-minute walking test,6MWT)。结果:治疗后,两组患者VAS、TUGT测试时间均显著降低,且治疗组显著低于同期对照组,差异均有统计学意义(P<0.05)。治疗后,两组患者ROM、HSS功能评分、6MWT均显著升高,且治疗组显著高于同期对照组,差异均有统计学意义(P<0.05)。结论:独活寄生汤联合常规康复训练对KOA患者TKA后早期运能影响较大,能改善患者TKA术后膝关节疼痛、提高关节活动度和下肢运动功能。

Objective:To observe the effect of Duhuo Jisheng Decoction combined with rehabilitation training on the early-stage motor function of patients with knee osteoarthritis(KOA)after total knee arthroplasty(TKA).Methods:90 KOA patients were divided into a control group and a treatment group by the random number table method,with 45 cases in each group.Both groups of patients underwent unilateral knee TKA.The control group was given daily routine rehabilitation training after the operation,while the treatment group was given Duhuo Jisheng Decoction on the basis of the treatment of the control group.Both groups were treated for 14 days.The visual analogue scale(VAS),range of motion(ROM)of the knee joints(range of motion,ROM),and hospital for special surgery(HSS),the time up and go test(TUGT),and the 6-minute walking test(6 MWT)of the two groups were compared before and 14 days after the treatment.Results:After treatment,the VAS and TUGT test time of the two groups were significantly reduced,and the treatment group was significantly lower than the control group during the same period,the difference was statistically significant(P<0.05).After treatment,the ROM,HSS knee score,and 6 MWT of the two groups were significantly increased,and the treatment group was significantly higher than the control group during the same period,the difference was statistically significant(P<0.05).Conclusion:Duhuo Jisheng Decoction combined with routine rehabilitation training has a significant effect on KOI patients,which can alleviate knee pain,and improve joint mobility and lower limb motor function after TKA.
