火针联合穴位封闭疗法治疗顽固性面瘫的临床观察

Observation on treating obstinate facial paralysis by fire acupuncture plus acupoint block therapy

目的:观察火针配合穴位封闭疗法治疗顽固性面瘫的临床效果。方法:按照抽纸条的方式,随机将72例顽固性面瘫患者分为两组,分别为对照组与观察组,每组36例,其中对照组运用电针配合穴位封闭疗法治疗,观察组采用火针配合穴位封闭疗法治疗,对比两组治疗效果以及治疗前后的症状改善情况,对面神经功能评分量表(House Brackmann,H-B)评分、面部残疾指数(Facial Disability Index,FDI)以及多伦多评价系统评分进行对比。结果:观察组临床总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后两组H-B分级中社会功能评分明显低于治疗前,且治疗后的观察组H-B分级评分明显低于对照组,差异有统计学意义(P<0.05)。两组治疗前患者FDI表评分差异不存在显著统计学意义(P>0.05),治疗后两组FDI中社会功能评分明显低于治疗前,且治疗后的观察组FDI中社会功能评分明显低于对照组,差异有统计学意义(P<0.05);治疗后两组FDI中躯体功能评分显著高于治疗前,且治疗后的观察组水平明显高于对照组,差异有统计学意义(P<0.05)。两组治疗前患者多伦多评价系统评分差异不存在显著统计学意义(P>0.05),治疗后两组多伦多评价系统评分明显高于治疗前,且治疗后的观察组高于对照组,差异有统计学意义(P<0.05)。结论:火针配合穴位封闭疗法治疗顽固性面瘫的效果显著,不仅对患者的症状有明显缓解,并且对面部神经的恢复和功能的恢复均有较好的作用和效果,也降低了面部残疾指数,在临床上可进行推广。

Objective:To observe the clinical effect of fire acupuncture plus acupoint block therapy on obstinate facial paralysis.Methods:According to the method of drawing paper strips,72 patients were randomly divided into the control group and the observation group,36 cases in each group.The control group was treated by electroacupuncture and acupoint block therapy,and the observation group was treated by fire acupuncture plus acupoint block therapy.Results:The total effective rate in the observation group was higher than the control group,and the difference was statistically significant(P<0.05).After treatment,the social function scores in H-B classification in two groups were significantly lower than before treatment,and the H-B classification score in the observation group was significantly lower than the control group(P<0.05).There was no significant difference in FDI scores between two groups before treatment(P>0.05).After treatment,the FDI scores in the two groups were significantly lower than before treatment,and those in the observation group were significantly lower than the control group,(P<0.05).There was no significant difference in the scores of the Toronto evaluation system between the two groups before treatment(P>0.05).After treatment,the scores of Toronto evaluation system in groups were significantly higher than before treatment,and that in the observation group was higher than the control group(P<0.05).Conclusion:Fire acupuncture plus acupoint block therapy on obstinate facial paralysis is effective,can significantly relieves the symptoms,recover facial nerves and functional recovery,and it also reduces the facial disability index,can be promoted clinically.
