正骨推拿联合针灸在退行性腰椎滑脱症患者中的应用及对其JOA评分的影响

Application of orthopedic massage combined with acupuncture in patients with degenerative spondylolisthesis and its effect on JOA score

目的:探讨正骨推拿联合针灸治疗退行性腰椎滑脱症(Degenerative Spondylolisthesis,DS)患者的疗效以及对其日本矫形外科学会(Japanese Orthopaedic Association,JOA)评分的影响。方法:选取佛山健翔医院2018年1月-2019年12月收治的112例DS患者,用随机数字表法将其分为观察组和对照组,各56例。对照组给予常规腰椎牵引+药物治疗,观察组在对照组的基础上加用正骨推拿联合针灸治疗。比较两组的临床效果、治疗前后的JOA评分及疼痛视觉模拟评分(Visual Analog Scale,VAS)。结果:观察组的临床总有效率为91.07%(51/56),较对照组的75.00%(42/56)高(P<0.05);两组治疗后JOA评分较同组治疗前明显升高,VAS评分明显降低,且观察组在治疗后各指标升高或降低的幅度明显高于对照组,差异具有统计学意义(P<0.05)。结论:正骨推拿联合针灸治疗DS可明显改善患者的临床症状、缓解疼痛,促进患者腰椎功能的恢复,疗效显著,值得临床推广。

Objective:To explore the curative effect of orthopedic massage combined with acupuncture on patients with degenerative spondylolisthesis(DS)and its effect on scores of Japanese Orthopaedic Association(JOA).Methods:A total of 112 DS patients who were admitted to Foshan Jianxiang Hospital from January 2018 to December 2019 were enrolled.They were divided into the observation group and the control group by random number table method,with 56 cases in each group.The control group was given routine lumbar traction and drugs,while observation group was additionally treated with orthopedic massage combined with acupuncture.The clinical effect,scores of JOA and visual analogue scale(VAS)before and after treatment were compared between the two groups.Results:The total clinical response rate of observation group was significantly higher than that of control group[91.07%(51/56)vs 75.00%(42/56)](P<0.05).After treatment,JOA scores were significantly increased,while VAS scores were significantly decreased in both groups.And the increase or decrease amplitude of each index in the observation group was significantly higher than that in control group(P<0.05).Conclusion:Orthopedic massage combined with acupuncture can significantly relieve clinical symptoms,reduce pain,and promote the recovery of lumbar function in the treatment of DS,it has a significant curative effect.
