RCA-CRRT治疗中滤器后离子钙与体内离子钙的相关性 被引量:1

Correlation between ionized calcium after filter and ionized calcium in vivo during RCA-CRRT treatment

摘要 目的研究在持续性肾脏替代治疗(CRRT)中采用局部枸橼酸抗凝(RCA)过程中滤器后血钙与体内血钙的相关性,寻找滤器后Ca2+管理的最适宜范围。方法回顾性分析危重患者行CRRT治疗的临床资料,在经验性补钙策略下,对比使用连续静脉-静脉血液透析滤过模式(CVVHDF)、RCA-CRRT过程中滤器后不同Ca2+水平下,患者体内离子钙不达标率。根据滤器后Ca2+管理目标为0.20~0.40 mmol/L,平均分为4组,组1滤器后Ca2+为0.20~0.249mmol/L组2滤器后Ca2+为0.25~0.299mmol/L组3滤器后Ca2+为0.30~0.349mmol/L组4滤器后Ca2+为0.35~0.4mmol/L。结果滤器后Ca2+与患者体内Ca2+呈正相关(r=0.535,P<0.05)。在治疗过程中,4组患者体内离子钙不达标率差异有统计学意义(P<0.05)。组1、组2不达标率高于组3、组4(均P<0.05)。4组患者出血发生率差异无统计学意义(P>0.05)。结论患者体内Ca2+水平随滤器后Ca2+的下降而减少,滤器后Ca2+与患者体内Ca2+存在正相关关系。在RCA-CRRT治疗行滤器后Ca2+管理时,把滤器后Ca2+控制在0.30~0.40mmol/L内是合理的。
