CK免疫组化联合EBER原位杂交在鼻咽癌诊断中的作用分析 被引量:1

摘要 目的CK免疫组化联合EBER原位杂交在鼻咽癌诊断中的作用。方法选取2016年1月—2019年12月在惠州市第三人民医院确诊为鼻咽癌的112例患者作为研究组,并选取同期于该院进行鼻咽活检诊断为鼻咽黏膜慢性炎症患者30例作为对照组,收集受试者的鼻咽活检组织,进行CK免疫组化及EBER原位杂交;比较两组患者鼻咽组织中CK免疫组化及EBER原位杂交阳性细胞出现的比例;观察鼻咽癌患者CK免疫组化及EBER原位杂交阳性细胞的状态;计算CK免疫组化及EBER原位杂交诊断鼻咽癌的临床效能。结果112例非角化型未分化癌患者的组织切片中,99例EBER原位杂交结果呈现阳性,诊断阳性率为88.39%;CK免疫组化结果显示,96例患者呈现阳性,诊断阳性率为85.71%,两组鼻咽活检组织中EBER及CK诊断阳性率比较,差异有统计学意义(χ^(2)=87.571、79.379,P<0.05);CK与EBER联合检测诊断鼻咽癌的灵敏度、特异性、准确性、阳性预测率及阴性预测率分别为97.32%、100.00%、97.89%、100.00%及90.91%,高于两种指标单独检测的灵敏度、准确率及阴性预测率,具有较高的临床效能。结论鼻咽癌患者的鼻咽活检组织中EBER原位杂交的染色程度较高,CK含量较高,能够通过两种检测指标的联合诊断对鼻咽癌进行诊断,发挥临床诊断作用。 Objective The role of CK immunohistochemistry combined with EBER in situ hybridization in the diagnosis of nasopharyngeal carcinoma.Methods A total of 112 patients diagnosed with nasopharyngeal carcinoma in Huizhou third people's hospital from January 2016 to December 2019 were selected as the research group,and patients with chronic nasopharyngeal mucosal inflammation diagnosed by nasopharyngeal biopsy in the hospital during the same period were selected 30 as a control group,the nasopharyngeal biopsy tissues of the subjects were collected,and CK immunohistochemistry and EBER in situ hybridization were performed;the ratio of CK immunohistochemistry and EBER in situ hybridization positive cells in the nasopharyngeal tissues of the two groups was compared;CK immunohistochemistry and EBER in situ hybridization positive cell status in patients with nasopharyngeal carcinoma;calculate the clinical efficacy of CK immunohistochemistry and EBER in situ hybridization in the diagnosis of nasopharyngeal carcinoma.Results Among 112 tissue sections of patients with non-keratinizing undifferentiated cancer,99 cases of EBER in situ hybridization showed positive results,with a positive diagnosis rate of 88.39%;CK immunohistochemistry results showed that 96 patients showed positive results with a positive diagnosis rate 85.71%,the positive rates of EBER and CK in the two groups of nasopharyngeal biopsy tissues were compared,and the difference was statistically significant(χ^(2)=87.571,79.379,P<0.05);the sensitivity,specificity,and sensitivity of combined detection of CK and EBER in the diagnosis of nasopharyngeal carcinoma accuracy,positive prediction rate and negative prediction rate were 97.32%,100.00%,97.89%,100.00%and 90.91%,respectively,which are higher than the sensitivity,accuracy and negative prediction rate of the two indicators tested separately,and have high clinical efficacy.Conclusion The staining degree of EBER in situ hybridization in nasopharyngeal biopsy tissue of patients with nasopharyngeal carcinoma is higher,and the content of CK is higher.It can diagnose nasopharyngeal carcinoma through the combined diagnosis of the two detection indicators and play a clinical diagnostic role.
