空心钉内固定和关节置换术治疗移位型股骨颈骨折的效果分析

Analysis of the Effect of Hollow Screw Internal Fixation and Joint Replacement in the Treatment of Displaced Femoral Neck Fractures

目的探讨空心钉内固定和关节置换术治疗移位型股骨颈骨折的效果。方法分析梅州市中医医院2016年1月—2019年12月收治的60例移位型股骨颈骨折患者临床资料,依据治疗方式不同进行分组,关节置换术组30例和空心钉组30例。观察两组患者手术时间、术中失血、住院时间、术后卧床时间、临床疗效、术后并发症情况。结果关节置换术组患者手术时间(71.5±8.5)min长于空心钉组(45.2±9.0)min,差异有统计学意义(t=11.636,P<0.05);术中失血量(140.6±16.7)mL多于空心钉组(60.2±12.3)mL,差异有统计学意义(t=21.232,P<0.05);住院时间(13.6±2.5)d和术后卧床时间(5.8±1.3)d均长于空心钉组(9.5±1.9)d、(4.1±1.4)d,差异有统计学意义(t=7.151、4.874,P<0.05);关节置换术组患者临床治疗优良率(96.7%)高于空心钉组(70.0%),差异有统计学意义(χ^(2)=7.680,P<0.05),两组患者术后并发症发生率比较,差异无统计学意义(P>0.05)。结论关节置换术治疗移位型股骨颈骨折患者,术后恢复明显,疗效好,并发症少,值得临床推广应用。

Objective To investigate the effect of hollow screw internal fixation and joint replacement in the treatment of displaced femoral neck fractures.Methods The clinical data of 60 patients with displaced femoral neck fractures admitted to Meizhou Hospital of Traditional Chinese Medicine from January 2016 to December 2019 were analyzed.According to different treatment methods,they were divided into groups,30 cases in the joint replacement group and 30 cases in the hollow nail group.The operation time,intraoperative blood loss,hospitalization days,postoperative bed time,clinical efficacy,and postoperative complications were observed in the two groups.Results The operation time of the joint replacement group was(71.5±8.5)min longer than that of the hollow nail group(45.2±9.0)min,the difference was statistically significant(t=11.636,P<0.05);and the blood loss during the operation was(140.6±16.7)mL more than that of the hollow nail group(60.2±12.3)mL,the difference was statistically significant(t=21.232,P<0.05);the length of hospital stay(13.6±2.5)d and the postoperative bed stay(5.8±1.3)d were longer than the hollow nail group(9.5±1.9)d,(4.1±1.4)d,the difference was statistically significant(t=7.151,4.874,P<0.05);joint replacement,the clinical treatment excellent and good rate(96.7%)of the patients in the operation group was higher than that of the hollow nail group(70.0%),the difference was statistically significant(χ^(2)=7.680,P<0.05);there was not statistically significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion Joint replacement for the treatment of displaced femoral neck fractures has obvious postoperative recovery,good curative effect and few complications.It is worthy of clinical application.
