计划性电话回访对类风湿关节炎患者服药依从性的影响

Effect of Planned Telephone Follow-up on the Compliance of Patients with Rheumatoid Arthritis

目的探讨计划性电话回访对类风湿关节炎患者服用甲氨蝶呤依从性的影响。方法选取2017年1月—2019年1月从扬州大学附属医院确诊,出院后服用甲氨蝶呤10 mg/qw的60例类风湿关节炎患者作为研究对象,随机分为对照组和观察组,每组30例。对照组采用常规电话回访,观察组由管床护士根据患者服用甲氨蝶呤的时间计划性回访。随访6个月后,对患者服药依从性及红细胞沉降率(erythrocyte sedimentation Rate,ESR)、C-反应蛋白(C-reactive protein,CRP)、类风湿因子(rheumatoid factor,RF)指标进行检查统计。结果对照组和观察组的服药依从率分别为53.33%和96.67%,观察组服药依从率显著高于对照组,差异有统计学意义(χ^(2)=15.022,P<0.001)。观察组ESR(12.96±5.57)mm/h、CPR(11.35±5.29)mg/L、RF(22.35±4.24)U/mL 3项类风湿指标均显著低于对照组(30.43±8.67)mm/h、(27.30±10.77)mg/L、(51.23±11.86)IU/mL,差异有统计学意义(t=9.285、7.280、12.559,P<0.001)。结论计划性电话回访能显著提高类风湿性关节炎患者服用甲氨蝶呤依从性,值得在临床推广应用。

Objective To investigate the effect of planned telephone follow-up visits on the compliance of patients with rheumatoid arthritis taking methotrexate.Methods Sixty patients with rheumatoid arthritis who were diagnosed from the Affiliated Hospital of Yangzhou University from January 2017 to January 2019 and were discharged from the hospital taking methotrexate 10 mg/qw were selected as the research subjects.They were randomly divided into a control group and an observation group.30 cases in each group.The control group used routine telephone return visits,and the observation group was scheduled to return visits by tube bed nurses according to the time when the patients took methotrexate.After 6 months of follow-up,the patient's medication compliance,erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),and rheumatoid factor(RF)indicators were checked and counted.Results The medication compliance rate of the control group and the observation group were 53.33%and 96.67%,respectively.The medication compliance rate of the observation group was significantly higher than that of the control group,the difference was statistically significant(χ^(2)=15.022,P<0.001).ESR(12.96±5.57)mm/h in the observation group;CPR(11.35±5.29)mg/L;RF(22.35±4.24)U/mL 3 rheumatoid indexes were significantly lower than those in the control group(30.43±8.67)mm/h;(27.30±10.77)mg/L;(51.23±11.86)U/mL,the difference was statistically significant(t=9.285,7.280,12.559,P<0.001).Conclusion Planned telephone follow-up visits can significantly improve the compliance of patients with rheumatoid arthritis in taking methotrexate,which is worthy of clinical application.
