腹腔镜卵巢囊肿剥除术与开腹手术治疗卵巢囊肿的临床疗效

Clinical effect of laparoscopic ovarian cystectomy and laparotomy in the treatment of ovarian cyst

目的分析腹腔镜卵巢囊肿剥除术与开腹手术治疗卵巢囊肿的效果。方法78例卵巢囊肿患者,随机分为对照组和观察组,每组39例。对照组给予开腹手术治疗,观察组给予腹腔镜卵巢囊肿剥除术治疗。比较两组手术效果(手术时间、术中出血量、住院时间),预后效果[肛门排气时间、术后月经恢复时间、视觉模拟评分法(VAS)评分],并发症发生率。结果观察组患者手术时间(52.80±5.43)min、住院时间(3.67±1.35)d短于对照组的(76.79±10.82)min、(6.82±1.76)d,术中出血量(48.34±11.06)ml少于对照组的(74.99±11.40)ml,差异有统计学意义(P<0.05)。观察组肛门排气时间(1.42±0.63)h、术后月经恢复时间(28.15±9.53)d短于对照组的(5.31±1.98)h、(36.79±9.53)d,VAS评分(1.35±0.65)分低于对照组的(3.25±1.32)分,差异有统计学意义(P<0.05)。观察组并发症发生率5.13%低于对照组的20.51%,差异有统计学意义(P<0.05)。结论给予卵巢囊肿患者腹腔镜卵巢囊肿剥除术治疗能够显著提升手术治疗效果和预后效果,且并发症发生率低,具有临床推广价值。

Objective To analyze the effect of laparoscopic ovarian cystectomy and laparotomy in the treatment of ovarian cyst.Methods A total of 78 patients with ovarian cysts were randomly divided into control group and observation group,with 39 cases in each group.The control group was treated with laparotomy,and the observation group was treated with laparoscopic ovarian cystectomy.The surgical effects(operation time,intraoperative blood loss,hospitalization time),prognostic effects[anal exhaust time,postoperative menstrual recovery time,visual analogue scale(VAS)score],and the incidence of complications were compared between the two groups.Results The operation time(52.80±5.43)min and hospitalization time(3.67±1.35)d of the observation group were shorter than(76.79±10.82)min and(6.82±1.76)d of the control group,and the intraoperative blood loss(48.34±11.06)ml was less than(74.99±11.40)ml of the control group.All the difference was statistically significant(P<0.05).The anal exhaust time(1.42±0.63)h and postoperative menstrual recovery time(28.15±9.53)d of the observation group were shorter than(5.31±1.98)h and(36.79±9.53)d of the control group,and the VAS score(1.35±0.65)points was lower than(3.25±1.32)points of the control group.All the difference was statistically significant(P<0.05).The incidence of complications in the observation group was 5.13%,which was lower than 20.51%in the control group,and the difference was statistically significant(P<0.05).Conclusion Laparoscopic ovarian cystectomy can significantly improve the surgical effect and prognostic effect of patients with ovarian cyst with low incidence of complications,and it contains clinical promotional value.
