轮廓化鼻内镜手术治疗难治性慢性鼻窦炎的疗效研究

Study on the efficacy of radical sinus surgery in the treatment of refractory chronic rhinosinusitis

目的研究轮廓化鼻内镜手术(RSS)治疗难治性慢性鼻窦炎的临床疗效。方法80例难治性慢性鼻窦炎患者,通过随机数字表法分为参照组和观察组,每组40例。参照组患者采用功能性鼻内镜手术进行治疗,观察组采用轮廓化鼻内镜手术治疗。比较两组患者临床疗效及鼻内镜检查量化评估评分法(Lund-Kennedy)评分、鼻窦CTLund-Mackay评分。结果参照组治疗总有效率为72.50%,观察组治疗总有效率为95.00%;观察组治疗总有效率显著高于参照组,差异有统计学意义(P<0.05)。观察组Lund-Kennedy评分(3.4±0.4)分、鼻窦CTLund-Mackay评分(6.2±0.7)分均低于参照组的(4.5±0.8)、(8.7±0.9)分,差异有统计学意义(P<0.05)。结论对难治性慢性鼻窦炎患者行轮廓化鼻内镜手术可以保证治疗效果,减轻患者临床症状,值得临床推广应用。

Objective To study the clinical efficacy of radical sinus surgery(RSS)in the treatment of refractory chronic rhinosinusitis.Methods A total of 80 patients with refractory chronic rhinosinusitis were divided into reference group and observation group according to random numerical table,with 40 cases in each group.The reference group was treated with functional endoscopic sinus surgery,and the observation group was treated with radical sinus surgery.The clinical efficacy,Lund-Kennedy score,and sinus CTLund-Mackay score were compared between the two groups.Results The total effective rate in the reference group was 72.50%,and the total effective rate in the observation group was 95.00%;the total effective rate in the observation group was significantly higher than that in the reference group,and the difference was statistically significant(P<0.05).The Lund-Kennedy score(3.4±0.4)points and the sinus CTLund-Mackay score(6.2±0.7)points of the observation group were lower than(4.5±0.8)and(8.7±0.9)points of the reference group,and the difference was statistically significant(P<0.05).Conclusion Radical sinus surgery can ensure the therapeutic effect and reduce the clinical symptoms of patients with refractory chronic rhinosinusitis,which is worthy of clinical promotion and application.
