正畸-牙周联合治疗前牙错颌畸形伴中重度牙周病的效果评价

Evaluation on the effect of combined orthodontic-periodontal therapy for malocclusion of anterior teeth with moderate-to-severe periodontal disease

目的观察正畸-牙周联合治疗前牙错颌畸形伴中重度牙周病的效果。方法60例前牙错颌畸形伴中重度牙周病患者,依据就诊的先后顺序分为对照组和观察组,每组30例。对照组采用单纯牙周病的基础治疗,观察组在对照组基础上采用正畸-联合牙周治疗。比较两组患者治疗前后牙周状况[探针深度(PD)、探针出血(BOP)、牙龈指数(GI)及菌斑指数(PLI)],疗效,牙齿松动度。结果治疗前,两组患者的PD、BOP、GI和PLI水平比较差异无统计学意义(P>0.05);治疗后,两组患者PD、BOP、GI和PLI水平均显著低于治疗前,且观察组PD、BOP、GI和PLI水平均显著低于对照组,差异具有统计学意义(P<0.05)。治疗后,观察组的总有效率93.33%显著高于对照组的73.33%,差异具有统计学意义(P<0.05)。治疗后,观察组牙齿松动0度患者占比60.00%显著高于对照组的33.33%,Ⅱ度患者占比6.67%和Ⅲ度患者占比6.67%均显著低于对照组的26.67%、26.67%,差异具有统计学意义(P<0.05)。结论正畸-牙周联合治疗前牙错颌畸形伴中重度牙周病能显著改善患者的口腔状况,提高治疗效果。

Objective To observe the effect of combined orthodontic-periodontal therapy for anterior open bite malocclusion with moderate-to-severe periodontal disease.Methods A total of 60 cases of anterior open bite malocclusion with moderate-to-severe periodontal disease were divided into control group and observation group according to visiting order,with 30 cases in each group.The control group received simple basic treatment of periodontal disease,and the observation group received combined orthodontic-periodontal therapy.The periodontal status[including probe depth(PD),bleeding on probing(BOP),gingival index(GI)and plaque index(PLI)]before and after treatment,clinical efficacy,and tooth looseness were compared between the two groups.Before treatment,there was no statistically significant difference in the levels of PD,BOP,GI and PLI between the two groups(P>0.05).After treatment,the levels of PD,BOP,GI and PLI of the two groups were significantly lower than those before treatment,and the levels of PD,BOP,GI and PLI of the observation group were significantly lower than those of the control group.All the difference was statistically significant(P<0.05).After treatment,the total effective rate 93.33%in the observation group was significantly higher than 73.33%in the control group,and the difference was statistically significant(P<0.05).After treatment,the proportion of patients with 0 degree of tooth looseness 60.00%in the observation group was significantly higher than 33.33%in the control group,and the proportion of patients with Ⅱ and Ⅲ degree of tooth looseness in the observation group were 6.67%and 6.67%,which were significantly lower than 26.67%and 26.67%in the control group.All the difference was statistically significant(P<0.05).Conclusion Combined orthodontic-periodontal therapy can significantly improved the oral condition of patients with anterior open bite malocclusion and moderate-to-severe periodontal disease,and improve their therapeutic effect.
