自锁矫治器系统在牙列拥挤非拔牙矫治中的临床应用研究

摘要 目的探讨自锁矫治器系统在牙列拥挤非拔牙矫治中的临床应用效果。方法选取60例于口腔门诊正畸科就诊的患者,均为Ⅰ度或Ⅱ度牙列拥挤,采用X拍摄方法测量分析软组织5项指标上唇倾角、颏唇沟角、面角、Z角、面突角,采用游标卡尺测量上颌各段牙弓宽度以及牙弓长度,分析上颌左右尖牙牙尖距离(WU3)、第一前磨牙中央窝间距(WU4)、第二前磨牙中央窝间距(WU5)、第一磨牙中央窝间距(WU6)。比较治疗前后头影测量结果及Ⅰ、Ⅱ度拥挤患者牙测量结果。结果60例患者治疗前后上唇倾角、面突角、颏唇沟角、面角、Z角比较,差异无统计学意义(P>0.05)。治疗后,Ⅰ度拥挤患者WU3、WU4、WU5、WU6及牙弓长度分别为(38.12±3.21)、(38.46±2.98)、(45.18±4.18)、(47.78±4.73)、(47.78±4.73)mm,均显著长于治疗前的(35.43±4.36)、(36.10±3.26)、(43.17±3.49)、(46.16±3.26)、(45.12±4.23)mm,差异均具有统计学意义(P<0.05)。治疗后,Ⅱ度拥挤患者WU3、WU4、WU5、WU6及牙弓长度分别为(38.26±2.89)、(38.48±3.31)、(45.78±4.16)、(49.27±3.46)、(48.60±3.44)mm,均显著长于治疗前的(35.89±3.45)、(36.68±3.15)、(43.13±3.26)、(47.26±3.19)、(45.24±2.24)mm,差异均具有统计学意义(P<0.05)。结论通过自锁矫正器系统对牙列拥挤非拔牙矫治后患者得到良好临床效果,值得临床推广应用。 Objective To discuss the clinical effect of self-locking appliance system in nonextraction correction of crowding dentition.Methods There were 60 patients in the Orthodontics Department of the stomatology clinic were selected,all of them had gradeⅠorⅡcrowding dentition.The X-shooting method was used to measure and analyze the five indexes of soft tissue,the upper lip inclination angle,the menthol sulcus angle,the face angle,the Z angle,and the face protrusion angle.Vernier calipers was used to measure the arch width and arch length of each segment of the upper jaw,and analyze the distance between the cusps of the left and right canines(WU3),the central fossa distance of the first premolar(WU4),the central fossa distance of the second premolar(WU5),and the central fossa distance of the first molar(WU6).The cephalometric measurements before and after treatment,and dental arch measurements in patients with grade Ⅰ and Ⅱ crowding dentition were compared.Results There was no statistically significant difference in the upper lip inclination angle,facial protrusion angle,menthol sulcus angle,facial angle and Z angle before and after treatment in 60 patients(P>0.05).After treatment,the WU3,WU4,WU5,WU6 and dental arch length in patients with grade Ⅰ crowding dentition were(38.12±3.21),(38.46±2.98),(45.18±4.18),(47.78±4.73),(47.78±4.73)mm,respectively,which were significantly longer than(35.43±4.36),(36.10±3.26),(43.17±3.49),(46.16±3.26),(45.12±4.23)mm before treatment,and the differences were statistically significant(P<0.05).After treatment,the WU3,WU4,WU5,WU6 and dental arch length of patients with grade Ⅱ crowding dentition were(38.26±2.89),(38.48±3.31),(45.78±4.16),(49.27±3.46),(48.60±3.44)mm,respectively,which were significantly longer than(35.89±3.45),(36.68±3.15),(43.13±3.26),(47.26±3.19),(45.24±2.24)mm before treatment,and the differences were statistically significant(P<0.05).Conclusion Damon orthodontic appliance has a good clinical effect on patients with non-extraction correction of crowding dentition,which is worthy of clinical promotion and application.
