摘要
目的探讨2型糖尿病患者铁代谢与胰岛功能的关系。方法随机选取2018年1月—2020年1月时间段该门诊收治的75例2型糖尿病患者作为该次研究主体,将其纳入治疗组,并根据患者病程长短,将其分为A组(病程<1年)、B组(病程1-10年)及C组(病程>10年),分别为24例,25例及26例,选取同期健康体检者75名作为对照组,检测治疗组与对照组的体质指数(BMI)、血红蛋白(Hb)、总胆固醇(TC)、血清低密度脂蛋白-胆固醇(LDL-C)、三酰甘油(TG)、血清铁蛋白、空腹胰岛素、空腹C肽水平,计算其胰岛素抵抗指数(HOMA-β及HOMA-IR),并将2组以上数据进行综合比较;对比A组、B组及C组空腹胰岛素、空腹C肽水平、胰岛素抵抗指数及血清铁蛋白水平,分析2型糖尿病患者病程长短铁代谢与胰岛功能的关系。结果两组TG、血清铁蛋白、空腹胰岛素、空腹C肽及HOMA-IR水平差异有统计学意义(P<0.05);3组空腹胰岛素、空腹C肽、血清铁蛋白及HOMA-IR水平差异有统计学意义(P<0.05);2型糖尿病患者血清铁蛋白水平与病程呈正相关,与空腹胰岛素、空腹C肽、HOMA-β及HOMA-IR呈负相关(P<0.05)。结论2型糖尿病患者的铁储备增加且和病程呈正相关,胰岛素及C肽与血清铁蛋白呈负相关,胰岛素抵抗指数与血清铁蛋白也呈负相关。铁负荷增加会导致胰岛素抵抗,加重糖尿病患者大血管病变。
Objective To investigate the relationship between iron metabolism and islet function in type 2 diabetic patients.Methods 75 patients with type 2 diabetes who were admitted to outheoutpatient clinic from January 2018 to January 2020 were randomly selected as the subjects of this study,and they were included in the treatment group,and according to the length of the disease course,they were divided into group A(disease course<1 year),B group(disease course 1-10 years)and C group(disease course>10 years),respectively 24 cases,25 cases and 26 cases,75 cases of healthy physical examination during the same period were selected as the control group,the treatment group and the control group of the body mass(BMI),hemoglobin(Hb),total cholesterol(TC),serum low-density lipoprotein-cholesterol(LDL-C),triglycerides(TG),serum ferritin,fasting insulin,fasting C-peptide were detected,calculate the insulin resistance index(HOMA-βand HOMA-IR),and compare the data of the above two groups comprehensively;compare the fasting insulin,fasting C peptide level,insulin resistance index and serum iron protein level in group A,group B and group C,to analyze the relationship between iron metabolism and pancreatic islet function in patients with type 2 diabetes.Results There were statistically significant differences in the levels of TG,serum ferritin,fasting insulin,fasting C-peptide and HOMA-IR between the two groups(P<0.05);the three groups had statistically significant differences in the levels of fasting insulin,fasting C-peptide,serum ferritin and HOMA-IR(P<0.05);Serum ferritin levels in patients with type 2 diabetes were positively correlated with the course of the disease,and negatively correlated with fasting insulin,fasting C-peptide,HOMA-βand HOMA-IR(P<0.05).Conclusion The iron reserves of patients with type 2 diabetes are increased and positively correlated with the course of the disease.Insulin and C-peptide are negatively correlated with serum ferritin,and insulin resistance index is also negatively correlated with serum ferritin,increased iron load can lead to insulin resistance and aggravate macrovascular disease in diabetic patients.
