分析血清铁蛋白与妊娠期糖尿病患者糖脂代谢的相关性研究 被引量:1

摘要 目的分析血清铁蛋白与妊娠期糖尿病患者糖脂代谢的相关性。方法纳入2019年1月—2020年12月收治的妊娠期糖尿病患者150例为观察组,同期选择150名健康体检孕妇为对照组进行对照研究,分析300例受检者孕周、年龄、体质指数、血脂、胰岛素指数等基础资料,采用Pearson分析血清铁蛋白与糖脂代谢相关性,采用Logistic回归分析法分析妊娠期糖尿病高危因素。结果单因素分析结果显示:观察组空腹血糖(FPG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbA1c)、三酰甘油(TG)、空腹胰岛素(Fins)、胰岛素抵抗指数(HOMA-IR)、血清铁蛋白等与对照组相比,差异有统计学意义(P<0.05)。多因素分析结果显示:血清铁蛋白、TG等诱发妊娠期糖尿病的高危因素。Pearson分析显示血清铁蛋白与TG、Fins、HOMA-IR、Hb A1c、FPG、2 hPG等呈正相关。结论血清铁蛋白水平的升高直接影响机体糖脂代谢水平,继而增加妊娠期糖尿病发生率,临床需加强对血清铁蛋白的控制,尽可能从根本上预防妊娠糖尿病的发生,确保母婴安全。 Objective To analyze the correlation between serum ferritin and glucose and lipid metabolism in patients with gestational diabetes.Methods 150 cases of gestational diabetes patients admitted from January 2019 to December 2020 were included as the observation group.During the same period,150 healthy pregnant women were selected as the control group for a controlled study.The gestational age,age,body mass index,basic data such as blood lipids and insulin index,Pearson was used to analyze the correlation between serum ferritin and glucose and lipid metabolism,and Logistic regression analysis was used to analyze high-risk factors of gestational diabetes.Results The results of univariate analysis showed that the observation group fasting blood glucose(FPG),2 h postprandial blood glucose(2 hPG),glycosylated hemoglobin(HbA1c),triacylglycerol(TG),fasting insulin(Fins),insulin resistance index(HOMA-IR),serum ferritin,etc.,compared with the control group,the difference was statistically significant(P<0.05).The results of multivariate analysis showed that serum ferritin,TG and other high-risk factors inducing gestational diabetes.Pearson analysis showed that serum ferritin was positively correlated with TG,Fins,HOMA-IR,Hb A1c,FPG,2 hPG,etc.Conclusion The increase of serum ferritin level will directly affect the body's glucose and lipid metabolism level,and then increase the incidence of gestational diabetes.In view of this clinical need to strengthen the control of serum ferritin,as far as possible to fundamentally prevent the occurrence of gestational diabetes,ensure the safety of mothers and babies.
