葛根芩连汤辨治睑板腺功能障碍性干眼

Gegen Qinlian decoction treats dry eye due to meibomian gland dysfunction

睑板腺功能障碍(MGD)性干眼病位在胞睑,按五轮、八廓辨证,病发于脾胃,以湿热内蕴证最为常见,中焦气机升降失常,湿热上犯于目,累及肝肺所主之黑睛、白睛,属中医津液输布异常致目窍干涩之证。葛根芩连汤是《伤寒论》经典名方,用于治疗太阳病桂枝证误下后邪热入里化热之协热下利,有解表清里、升清止利之功。在此基础上加以发挥,临证运用葛根芩连汤治疗湿热内蕴型MGD性干眼,取其升清降浊,以斡旋中焦气机,持中央运四方,消五方之患。本文基于中医对干眼及MGD性干眼的认识、葛根芩连汤的功效主治,探讨葛根芩连汤治疗湿热内蕴型MGD性干眼的思路及方法,以期为临床中医论治本病提供新途径。

The disease location of the meibomian gland dysfunction(MGD)dry eye is in the eyelid,according to the five wheel,eight belts differentiation,the disease occurs in the spleen and stomach,with damp heat syndrome is the most common one,up-bearing and down-bearing disorder of qi activity in middle-energizer,damp-heat invades the eyes upward,involving the dark and white of the eye dominated by liver and lung,which belongs to the syndrome of dry eye caused by abnormal body fluid delivery.Gegen Qinlian decoction is a classic prescription in Shanghanlun.It is used to treat diarrgea with heat caused by intrinsic heat transformed by evil heat,which is caused by cassia twig syndrome of Taiyang disease after wrong purgation methods.It has the functions of releasing exterior and clearing interior,upbearing the clear and checking diarrhea.On this basis,clinical application of Gegen Qinlian decoction in the treatment of dry eye due to MGD with dampness and heat accumulation,with its function of ascending lucidity and descending turbidity,which is mediating the qi activity of middle energizer,holds the central movement to dominant the other four directions,and eliminates the trouble of five directions.Based on the understanding of dry eye and dry eye due to MGD in Traditional Chinese Medicine and the efficacy and indications of Gegen Qinlian decoction,this paper discusses the thinking and methods of Gegen Qinlian decoction in the treatment of MGD dry eye with damp heat accumulation,in order to provide a new way for clinical treatment of the disease.
