老年脑梗死患者住院费用病例分组分析

Analysis of Hospitalization Expenses of Elderly Patients with Cerebral Infarction

目的:合理预测DRG病例分组的费用,为医保支付决策提供参考依据。方法:通过病案编码系统检索2016—2020年脑梗死老年住院患者首页信息,采用单因素和多重线性回归分析筛选影响住院费用节点变量,构建决策树模型对住院费用进行病例分组分析。结果:治疗方式、合并症伴随病、转归、费用类别是影响脑梗死老年患者住院费用主要自变量以及回归树的重要分类节点变量。结论:脑梗死老年患者住院费用的病例分组组内同质性和组间异质性较好,分组合理,能够为医保支付改革提供一定参考依据。

Objective:To analyze the hospitalization expenses of elderly patients with cerebral infarction by E-CHAID.Methods:The home page information of elderly inpatients with cerebral infarction from 2016 to 2020 is extracted through the medical record information system.Single factor analysis and multiple linear regression analysis are used to screen the classification node variables influencing the hospitalization expenses.Decision tree is constructed to analyze the hospitalization expenses by case grouping.Results:Treatments,comorbidities,outcomes and cost categories are the main independent variables affecting the hospitalization expenses of elderly patients with cerebral infarction and the important classification node variables of the regression tree.Conclusion:The intra group homogeneity and inter group heterogeneity of DRG in hospitalization expenses are better,and DRG grouping is reasonable,which provides certain references for medical decision-making and medical insurance payment reform.
