摘要
目的分析256层螺旋CT血管造影检查(CTA)对冠状动脉狭窄及不同性质斑块的诊断价值。方法从2019年7月—2021年1月该院收治的冠状动脉狭窄患者中方便选取77例进行研究,均采用256层螺旋CTA进行诊断,将冠状动脉血管造影(CAG)结果作为金标准,分析不同性质斑块与冠状动脉狭窄程度情况,斑块分布,CTA诊断的敏感度、特异度、准确度。结果CTA诊断冠状动脉狭窄轻度占比50.65%(39/77),中度占比31.17%(24/77),重度占比18.18%(14/77);CTA诊断右冠斑块分布占比12.99%(10/77),左冠斑块分布占比(38.96%)(30/77),前降支斑块分布占比32.47%(25/77),左旋支斑块分布占比15.58%(12/77);将CAG诊断结果作为金标准,分析CTA诊断冠状动脉轻度狭窄的敏感度、特异度、准确度分别为97.41%、97.37%、93.51%;中度狭窄敏感度、特异度、准确度分别为(83.33%)、90.57%、88.31%;重度狭窄敏感度、特异度、准确度分别为57.14%、96.83%、89.61%。结论256层螺旋CTA可诊断不同性质斑块在冠状动脉中的分布情况,且对不同程度冠状动脉狭窄诊断准确性较高,因此,该方式能为临床诊断及治疗提供科学依据。
Objective To analyze the diagnostic value of 256-slice spiral CT angiography(CTA)for coronary artery stenosis and plaques of different nature.Methods A total of 77 patients with coronary artery stenosis admitted to the hospital from July 2019 to January 2021 were conveniently selected for the study.They were all diagnosed with 256-slice spiral CTA,and coronary angiography(CAG)results were used as the gold standard to analyze different properties.The degree of plaque and coronary artery stenosis,plaque distribution,sensitivity,specificity,and accuracy of CTA diagnosis.Results CTA diagnosis of coronary artery stenosis accounted for 50.65%(39/77),moderate accounted for 31.17%(24/77),severe accounted for 18.18%(14/77);CTA diagnosed the proportion of right coronary plaque distribution 12.99%(10/77),the proportion of left coronary plaque distribution 38.96%(30/77),the proportion of anterior descending branch plaque distribution(32.47%)(25/77),the proportion of left circumflex branch plaque distribution 15.58%(12/77);the CAG diagnosis result as the gold standard,the sensitivity,specificity,and accuracy of the analysis of CTA in the diagnosis of mild coronary artery stenosis were 97.41%,97.37%,and 93.51%,respectively;the sensitivity,specificity,and accuracy of moderate stenosis were 83.33%and 90.57%,88.31%;the sensitivity,specificity,and accuracy of severe stenosis were 57.14%,96.83%,and 89.61%,respectively.Conclusion 256-slice spiral CTA can diagnose the distribution of different types of plaques in the coronary arteries,and the accuracy of the diagnosis of different degrees of coronary artery stenosis is high.Therefore,this method can provide scientific basis for clinical diagnosis and treatment.
