地佐辛对瑞芬太尼复合七氟醚麻醉患者的影响

The Effect of Dezocine on Patients Anesthetized With Remifentanil Combined With Sevoflurane

目的探究对瑞芬太尼复合七氟醚麻醉患者使用地佐辛后,患者恢复期躁动的具体表现及药物机制。方法随机选取2017年12月—2019年9月于我院接受治疗的瑞芬太尼复合七氟醚麻醉患者共142例,依据术后静脉注射方案的不同,将其分为观察组和对照组两组,每组71例;对照组患者术后静脉注射生理盐水,观察组患者术后静脉注射地佐辛,比较两组患者术后恢复期具体表现。结果观察组T1、T2时刻血氧饱和度明显高于对照组,差异有统计学意义(P<0.05);T2、T3时刻心率明显低于对照组,差异有统计学意义(P<0.05);T1、T2时刻平均动脉压明显低于对照组,差异有统计学意义(P<0.05)。术后苏醒期60 min内,观察组患者T2、T3时刻Ramasy评分明显高于对照组,差异有统计学意义(P<0.05)。两组患者苏醒期躁动评分,观察组明显低于对照组(t=5.151,P=0.000);观察组躁动发生率明显低于对照组(χ2=4.508,P=0.034)。结论地佐辛可降低瑞芬太尼复合七氟醚患者术后苏醒期的躁动发生率,几乎无呼吸抑制作用,具备安全性。

Objective To explore the specific manifestations and drug mechanism of restlessness during the recovery period of patients with remifentanil combined with sevoflurane anesthesia.Methods A total of 142 patients anesthetized with remifentanil combined with sevoflurane were randomly selected from December 2017 to September 2019 in our hospital.According to the different postoperative intravenous injection schemes,they were divided into observation group and control group Two groups,71 cases in each group;patients in the control group received intravenous injection of normal saline after the operation,and the observation group received intravenous injection of dezocine after the operation.The specific performance of the two groups during the postoperative recovery period were compared.Results The blood oxygen saturation of the observation group at T1 and T2 was significantly higher than that of the control group(P<0.05);the heart rate at T2 and T3 was significantly lower than that of the control group(P<0.05);the average arterial pressure at T1 and T2 was significantly lower than the control group(P<0.05).Within 60 minutes of postoperative recovery period,the ramsy scores of patients in the observation group at T2 and T3 were significantly higher than those in the control group(P<0.05).The agitation scores of the two groups of patients during the recovery period were significantly lower in the observation group than in the control group(t=5.151,P=0.000);the incidence of agitation in the observation group was significantly lower than that in the control group(χ2=4.508,P=0.034).Conclusion Dezocine can reduce the incidence of restlessness in patients with remifentanil combined with sevoflurane during the postoperative recovery period.It has almost no respiratory depression and is safe.
