冠心病合并糖尿病冠脉CT成像的应用研究

Application Research of Coronary CT Imaging in Coronary Heart Disease Complicated with Diabetes

目的研究CT成像应用于冠心病合并糖尿病患者的临床价值。方法选取2018年10月—2019年10月该院收治的45例冠心病合并糖尿病患者作为研究对象,并纳入观察组,另选同期该院收治的45例糖尿病怀疑冠心病患者作为对照组研究对象;90例研究对象均接受CT检查,对比两组患者冠脉CT成像特点。结果对比两组患者冠脉多支病变情况,观察组冠脉多支病变检出23例,发生率51.11%,对照组冠脉多支病变检出12例,发生率26.67%,差异有统计学意义(χ^(2)=5.657,P=0.017)。对比两组患者发生弥漫性病变情况,观察组弥漫性病变检出13例,发生率28.89%,对照组弥漫性病变检出5例,发生率11.11%,差异有统计学意义(χ^(2)=4.444,P=0.035)。对比两组患者病变血管斑块钙化情况,观察组病变血管斑块钙化检出33例,斑块钙化率66.67%,对照组病变血管斑块钙化检出24例,斑块钙化率53.33%,差异有统计学意义(χ^(2)=3.875,P=0.048)。结论CT可有效监测冠心病合并糖尿病患者的冠脉病变程度及斑块性质,可作为糖尿病患者怀疑冠心病的检查手段。

Objective To study the clinical value of CT imaging in patients with coronary heart disease and diabetes.Methods 45 patients with coronary heart disease and diabetes mellitus admitted to the hospital from October 2018 to October 2019 were selected as the research objects and included in the observation group.Another 45 patients with diabetic coronary heart disease admitted to the hospital during the same period were selected as the control group's research subjects.90 subjects underwent CT examination,and the characteristics of coronary CT imaging were compared between the two groups.Results Compared the two groups of patients with multivessel coronary artery disease,23 cases of coronary artery multivessel disease were detected in the observation group,with an incidence rate 51.11%,and 12 cases of coronary artery multivessel disease were detected in the control group,with an incidence rate 26.67%,the differences are statistically significant(χ^(2)=5.657,P=0.017).Compared the two groups of patients with diffuse lesions,13 cases of diffuse lesions were detected in the observation group with an incidence 28.89%,and 5 cases with diffuse lesions in the control group were detected with an incidence 11.11%,the differences was statistically significant(χ^(2)=4.444,P=0.035).Compared the calcification of diseased vascular plaque in the two groups,33 cases of diseased vascular plaque calcification were detected in the observation group,with a plaque calcification rate of 66.67%,and 24 cases of diseased vascular plaque calcification in the control group were detected with a plaque calcification rate of 53.33%,the differences was statistically significant(χ^(2)=3.875,P=0.048).Conclusion CT can effectively monitor the degree of coronary artery disease and the nature of plaque in patients with coronary heart disease and diabetes,and it can be used as a means of examination for suspected coronary heart disease in diabetic patients.
