富血小板血浆缓解膝骨性关节炎慢性疼痛的短期疗效评估

Short-term effect of platelet-rich plasma on chronic pain in patients with knee osteoarthritis

目的探讨富血小板血浆(PRP)缓解膝骨性关节炎慢性疼痛的短期疗效。方法抽取2019年1月至2019年12月漯河市第三人民医院诊断为膝骨性关节炎的住院患者20例(40个膝关节),依据Kellgren-Lawrence(K-L)分类方法分为2~4级,给予PRP注射,每周1次,连续3周,于第0天及第3、6、12周使用静息和活动视觉模拟量表(VAS)评估疼痛状况。结果20例患者中男3例,女17例,年龄(53.60±9.12)岁,体质指数(27.4±8.62)kg/m^(2)。20例患者双膝关节受累,40膝中K-L 2级14膝,K-L 3~4级26膝。第12周静息、活动VAS评分低于第0天的相应值(P<0.05),K-L 2级患者的第0天和第3周静息、活动VAS评分低于第3~4级(P<0.05)。结论PRP注射在12周内对膝骨性关节炎患者膝关节慢性疼痛有明显改善作用。

Objective To investigate the short-term effect of platelet-rich plasma(PRP)on chronic pain in patients with knee osteoarthritis.Methods Twenty inpatients(40 knee joints)who were diagnosed with knee osteoarthritis from January 2019 to December 2019 in Luohe Third People’s Hospital were selected,according to the Kellgren-Lawrence(KL)classification method,they were divided into grade 2-4 knee based on the Kellgren-Lawrence(K-L)classification,completed 3 doses of intra-articular PRP injections administered at intervals of 1 weeks.The patients’pain was evaluated using a resting and activity visual analog scale(VAS)on day 0,and week 3,6,and 12.Results Of the 20 patients,3 were males and 17 were females,aged(53.60±9.12)years old,and body mass index was(27.4±8.62)kg/m^(2).A total of 20 patients had bilateral knee involvement,and 26 of the 40 knee joints were classified as K-L grade 3-4 OA,14 as K-L grade 2 OA.The resting and active VAS scores at the 12th week were lower than the corresponding values on the 0th day(P<0.05).The resting and active VAS scores on the 0th and 3rd weeks of the K-L grade 2 patients were lower than that of the K-L grade 3-4(P<0.05).Conclusions PRP injections provided a meaningful improvement in chronic knee pain in patients with knee osteoarthritis throughout a 12-week period.
