腮腺炎病毒感染对成年男性精液质量的影响

Effect of mumps virus infection on the semen quality of adult men

目的探讨感染腮腺炎病毒(MuV)但未并发急性腮腺炎性睾丸炎的男性在育龄时的精液质量与未感染者的差异。方法采用回顾性队列研究,选择于我科进行孕前检查的1022名育龄男性为研究对象,分析年龄、体质量指数(BMI)、MuV感染史、吸烟量、基础病史对精液参数的影响。按是否曾经感染MuV,分为感染组(n=56)和未感染组(n=963),比较2组间各项精液参数的差异。结果感染组男性的精子总数、精子浓度、前向运动精子百分率(PR)、正常形态精子百分率(NSMR)、精子DNA碎片指数(DFI)、精子高DNA可染性(HDS)均差于未感染组(P<0.05)。MuV感染史与精子总数、精子浓度、NSMR呈负相关(P<0.01),与DFI、HDS呈正相关(P<0.01)。多因素回归分析结果显示,MuV感染史对精子总数、精子浓度、PR、NSMR、DFI、HDS存在负性影响,有统计学意义(P<0.05)。结论MuV感染与精子质量异常有明显的相关性。对于无急性睾丸炎症状的MuV感染者,要格外关注睾丸的慢性损害对远期生育力的影响。

Objective To explore the differences in the semen quality at childbearing age between men who were infected with mumps virus(MuV)but did not have acute mumps orchitis and those who were not infected.Methods In this retrospective cohort study,1022 men at childbearing age who had undergone pre-pregnancy check-ups at our department were selected.The effects of age,body mass index(BMI),MuV infection history,smoking history,and basic medical history on various semen parameters were analyzed.According to the MuV infection history,the subjects were divided into the infected group(n=56)and the non-infected group(n=963).The differences in each semen parameter were compared between the two groups.Results The total sperm count,sperm concentration,progressive motility(PR),normal sperm morphology rate(NSMR),DNA fragment index(DFI),and high DNA stainability(HDS)of the infected group were all worse than those of the non-infected group(P<0.05).MuV infection history was negatively correlated with the total sperm count,sperm concentration,and NSMR(P<0.01),and positively correlated with DFI and HDS(P<0.01).The results of multivariate regression analysis showed that MuV infection history had a negative impact on the total sperm count,sperm concentration,PR,NSMR,DFI,and HDS,and the results were statistically significant(P<0.05).Conclusion MuV infection is related to abnormal sperm quality.We should pay attention to the impact of chronic testicular damage on long-term fertility in patients with MuV infection but without acute testicular symptoms.
